Abstract
Relaksacijska je udlaga mobilna intraoralna naprava koja se rutinski primjenjuje pri liječenju temporomandibularnih poremećaja (TMP) i neželjenih posljedica bruksizma, a osigurava točkaste kontakte s antagonističkim zubima u centričnoj relaciji (CR) te osigurava vođenje očnjakom u laterotruzijskim kretnjama. Budući da je mehanizam djelovanja udlaga i dalje nepoznat, svrha je rada opisati terapijski koncept djelovanja te prikazati primjenu relaksacijske udlage u kliničkom radu. Najčešće je indicirana kod mialgije žvačnih mišića, artralgije temporomandibularnog zgloba (TMZ) te zaštite zuba od prekomjernog trošenja pri bruksističkim aktivnostima. Udlagom se postiže stabilniji ortopedski položaj kondila te samim time i normalizacija mišićnog tonusa što pridonosi otklanjanju simptoma. S obzirom na to da je izrada relativno jednostavna, da je terapijski pristup neinvazivan i bez posebnih kontraindikacija, relaksacijska udlaga pokazala se idealnim terapijskim sredstvom u inicijalnoj fazi otklanjanja znakova i simptoma temporomandibularnih poremećaja.Michigan splint is a removable intraoral appliance which is routinely used in the treatment of bruxism and temporomandibular disorders. It provides occlusal contacts between teeth in centric relation and canine guidance in laterotrusal movements. Since the mechanism of the splint action is still unclear, the aim of this paper is to describe therapeutic concept and indications of the Michigan splint in clinical practice. The most common indications for its use are myalgia of the masticatory muscles, arthralgia of the temporomandibular joint and protection of dental tissue from excessive wear during bruxism activity. The splint provides stable orthopedic condylar position, and normalization of muscular tone which contributes to the eliminations of the symptoms. With relatively simple fabrication, non-invasive treatment method and without any special contraindications, the Michigan splint has proven to be ideal therapeutic device for the initial treatment and elimination of the signs and symptoms of temporomandibular disorders
