P300 WAVE CHANGES IN PATIENTS WITH PARKINSON'S DISEASE

Abstract

CILJ ISTRAŽIVANJA: Dokazati postoje li statistički značajne razlike u duljini latencije vala P300 ciljanog i neciljanog podraţaja i amplitudi vala P300 ciljanog i neciljanog podražaja između ispitanika oboljelih od Parkinsonove bolesti u usporedbi s referentnim vrijednostima za zdravu populaciju. ISPITANICI I METODE: Studija je dizajnirana kao presječno istraživanje. Provedena je u Referentnom centru za evocirane potencijale ministarstva zdravlja Republike Hrvatske, Klinici za neurologiju KBC-a Split. . Podatci su prikupljani u vremenu od 1. svibnja 2015. godine zaključno s 1. siječnja 2016. godine. U ispitivanje je uključen 21 ispitanik obolio od Parkinsonove bolesti. Nakon pregleda medicinske dokumentacije te analize kriterija uključenja i isključenja, ispitanici su na jednak način podvrgnuti ispitivanju slušnog kognitivnog potencijala (val P300) nakon čega su dobiveni rezultati analizirani i uspoređivani s referentnim vrijedostima za zdravu populaciju. REZULTATI: Dokazali smo statistički značajnu razliku medijana duljine latencije vala P300 ciljanog podražaja između ispitanika oboljelih od Parkinsonove bolesti i referentnih vrijednosti za zdravu populaciju. Medijan duljine latencije vala P300 ciljanog podražaja u ispitanika s Parkinsonovom bolesti bio je 314.76 ms (299-326) dok je kao referentni interval uzeta vrijednost od 300± 10 ms. Također smo dokazali statistički značajnu razliku medijana duljine latencije vala P300 neciljanog podražaja. Medijan duljine latencije vala P300 neciljanog podražaja u ispitanika s Parkinsonovom bolesti iznosio je 345 ms (319-353), a referentni interval 300± 10 ms, kao i kod ciljanog podraţaja. Analizirajući amplitudu vala P300 ciljanog podražaja kod ispitanika oboljelih od Parkinsonove bolesti od 21 ispitanika njih 18 je imalo patološke vrijednosti. Što se tiče amplitude vala P300 neciljanog podražaja, od 21 ispitanika, 20 njih je imalo patološki nalaz amplitude neciljanog podražaja. ZAKLJUČCI: Bolesnici oboljeli od Parkinsonove bolesti imaju produljenu latenciju vala P300 kako ciljanog tako i neciljanog podražaja u odnosu na referentne vrijednosti za zdravu populaciju. Osim produljene latencije imaju i sniženu amplituda vala P300 ciljanog i neciljanog podražaja u usporedbi s referentnim intervalom zdrave populacije.OBJECTIVES: Show whether there is a statistically significant difference in length of the P300 wave latency of targeted and untargeted stimulus and statistically significant difference in the P300 wave amplitude of targeted and untargeted stimulus between group of patients with Parkinson's disease compared to reference value for healthy population. PATIENTS AND METHODES: The study was designed as a cross-sectional study. Conducted at the Reference Centre for evoked potentials Ministry of Health of the Republic of Croatia, Department of Neurology, University Hospital Split. Data were collected in the period from May 1st 2015. until January 1st 2016. The study included 21 patient suffering from Parkinson's disease. After reviewing the medical records and analyzes the inclusion and exclusion criteria, patients were subjected to the same procedure examining auditory cognitive potentials (P300 wave) and the results were analyzed and compared to reference value for healthy population. RESULTS: We demonstrate a statistically significant difference in the median length of the P300 wave latency of target stimulus between group of patients with Parkinson's disease compared to reference value for healthy population. The median length of the P300 wave latency of target stimulus in patients with Parkinson's disease was 314.76 ms (299-326) until as the reference value for healthy population was taken value of 300± 10 ms. We have olso shown that there is a statistically significant difference of the median length of the P300 wave latency of untargeted stimulus. The median length of the P300 wave latency of untargeted stimulus in patients with Parkinson's disease was 345 ms (319-353) and reference value for healthy population was 300± 10 ms just like one in the P300 wave latency of target stimulus. By analyzing the P300 wave amplitude of target stimulus in the group of patients suffering from Parkinson's diseas of 21 patient, 18 had abnormal findings. Regarding the P300 wave amplitude of untargeted stimulus, of 21 patient, 20 of them had abnormal findings. CONCLUSIONS: Patients suffering from Parkinson's disease have prolonged the P300 wave latency both of targeted and untargeted stimulus compared to reference value for healthy population. Except for prolonged latency, they have reduced the P300 wave amplitude of targeted and untargeted stimulus compared to reference value for healthy population
