IntroductionThe zygomycetes are a class of fungi that are ubiquitous insoil and decaying vegetable matter but can cause a varietyof infections in humans. The genera most commonly foundin human infections are Rhizopus and Mucor [1]. Infectiontypically occurs in immunosuppressed patients such asthose with transplants, malignancies, and diabetes mellitusand is usually associated with rhino-orbital-cerebral andpulmonary disease [2]. Infection of the skin and soft tissueswith zygomycetes is the third most common diagnosis aftersinus and pulmonary forms [1]. Cutaneous zygomycosisresults from inoculation of the spores into disrupted dermisand can occur in patients with little or no underlyingimmunodeficiency [3–5].2. Case ReportA 50-year-old woman was referred to the Infectious DiseaseClinic for nonhealing painful hand wounds, twelve days aftersustaining dog bites on the dorsum of both hands. She had ahistory of insulin-dependent diabetes mellitus for two yearsand hypertension. She was initially evaluated at an urgentcare center in Rhode Island on the day of the dog bite. At thattime, she was noted to have open wounds on the dorsum ofboth hands which were subsequently cleaned and stitched.She was given a fourteen-day course of amoxicillin. Six daysafter the initial dog bite, she returned to the urgent carecenter to have the stitches removed. At that time, she wasnoted to have an enlarged, draining nodule on the left hand.As a result, the stitches were only removed from the righthand. The bloody drainage from the left hand wound wassent for culture. Three days later, nine days after initial dogbite, she was called back to the urgent care center and wastold mold and bacteria grew in the culture and that the moldwas likely a contaminant. At this visit, a small eschar replacedthe draining nodule on the dorsum of her left hand. Thestitches were removed from the left hand and a black, stringymaterial was noted in the wound. This material was sent tothe microbiology laboratory for culture, and the wound waspacked. The patient continued her amoxicillin and elevendays after the dog bite, she returned to the urgent care centerfor the fourth time and once again was told that the culturegrew the same bacteria and mold, which was later identifiedas Enterobacter cloacae and Mucor species (Figure 1). Thepatient was referred to the Infectious Disease Clinic.The patient denied any other unusual exposures follow-ing the dog bites. She denied fever or chills. Her currentmedications were insulin, metformin, aspirin, and lisinopril.She had no known drug allergies. At the Infectious DiseaseClinic, her exam was remarkable for normal vital signsand bilateral hand ulcers, left greater than right (Figure 2).The patient was sent from the Infectious Disease Clinic tothe Emergency Department (ED) in order to be evaluated2 Case Reports in Infectious DiseasesFigure 1: Microscopic sample obtained from patient’s wounds. We recommended that the patient receivemeropenem 1mg/kg and 5mg/kg of amphotericin B (lipidcomplex; Abelcet; Enzon Pharmaceuticals, Bridgewater, NJ,USA).In the ED, the patient’s white blood cell (WBC) was7900/uL, with a normal diﬀerential and normal chemistrypanel. Her hemoglobin A1c was 7.8%. Blood cultures weredrawn, which were negative for growth. Within three hoursof arrival to the ED, the patient was taken to the OperatingRoom (OR) for debridement of left dorsal hand wound. Thesurgeons debrided down through the subcutaneous tissue tothe level of the peritenon, which are the connective tissuestructures attached to and surround the tendon (Figure 3).The specimen was sent to pathology and microbiology. Thistime, the cultures did not grow. The pathologist foundchronic active ulcer and organizing abscess in the surgicalspecimen.Three days after surgical debridement, the patient’swound was healing well, and her intravenous antibiotics wereswitched to ciprofloxacin 500mg orally every twelve hoursfor two weeks and posaconazole 100mg orally every twelvehours for two weeks.Figure 3: Patient’s left hand following surgical debridement.3. DiscussionThe presence of more than one organism isolated from awound following a dog bite is not unusual, but the presenceof Mucor sp is atypical. The most common bacteria found ina dog’s mouth include: Staphylococcus species, Streptococcusspecies, Eikenella species, Pasteurella species, Proteus species,Klebsiella species, Enterobacter species, and Capnocytophagacanimorsus [6]. Thus, the Enterobacter infection likely camefrom bacteria already in the dog’s mouth. We postulate thatthis patient’s fungal infection may have resulted from thedog’s mouth being contaminated with soil that containedMucor sp. Alternatively, the patient could have contaminatedher wound with soil from her yard. However, the latter seemsless likely, as the patient did not recall putting her handsin soil after the dog bite. Infection with zygomycetes hasalso been associated with contaminated traumatic wounds,dressings and splints, burns, and surgical sites [3–5]. Thiscase is significant, because it is diﬃcult to find case reportsin the literature of polymicrobial infection involving Mucorsp. following a dog bite. Additionally, previous case reportsof cutaneous zygomycosis involve patients with greaterimmunosuppression [7, 8] although cutaneous mucormyco-sis can occur in patients with little or no immunodeficiency[9]. Our patient had diabetes, but her glucose level wasmoderately controlled with a hemoglobin A1c less than 8%.The development and diagnosis of cutaneous zygomy-cosis was also delayed in this case compared to its morerapid development in immunosuppressed patients [7, 8].The diagnosis is often made on clinical grounds and canbe very challenging; tissue biopsy for histology and cultureis usually needed [3]. The gross appearance of skin lesionsinfected with Mucor sp. varies but most often causes anulcerative lesion with eschar formation [10], as it did in ourpatient. Lesions consistent with cutaneous zygomycosis werefirst noted 9 days after the patient’s dog bite. The first timeMucor sp was isolated from our patient, it was thought to bea contaminant and thus was not treated immediately, causinga delay in diagnosis and appropriate treatment. Findingthe same organism twice did prompt medical personnel toreconsider the diagnosis of fungal infection in this patientwho was not responding to appropriate medical therapy.Case Reports in Infectious Diseases 3The best outcomes from cutaneous zygomycosis havebeen associated with early detection, aggressive surgicaldebridement, early use of eﬀective antifungal therapy, andcorrection of predisposing factors [11]. Although intra-venous amphotericin B (lipid formulation) is the drug ofchoice initially, oral posaconazole is used as step-downtherapy for patients who have responded to amphotericinB [12]. Despite early diagnosis and aggressive combinedsurgical and medical therapy, the prognosis for recoveryfrom zygomycosis is poor with the exception of cutaneousinvolvement, which rarely disseminates [11]. First-line oralantibiotic therapy following a dog bite is amoxicillin-clavulanic acid for 10 days, which was appropriately given tothe patient. Unfortunately, amoxicillin-clavulanic acid is noteﬀective against mold, and thus did not treat our patient’sfungal infection. Diagnosis of Mucor sp as a pathogen in thewound infection would have resulted in a more appropriateantibiotic earlier in her treatment course.4. ConclusionIn conclusion, this case highlights the importance of dili-gence in collecting microbiologic data when there is minimalclinical response to empiric antibiotics. In any patient, wherethere is a possibility of soil contamination and the presenceof an advancing necrotic indurated lesion, consideration ofzygomycosis as the diagnosis should be made. It is importantto consider the microbiologic results before dismissing themas contamination and recognize mold as an important andpotential pathogen. The patient was taking amoxicillin forseveral days without clinical improvement, and it becamecritical that cultures were taken in order to establish thediagnosis. Lastly, this patient received a referral from theurgent care center to an infectious disease specialist andhad a surgical evaluation and debridement. Because all ofthese critical steps (obtaining cultures, infectious disease, andsurgical consultations) were taken during the care of thispatient, she had a good outcome.References[1] M. M. Roden, T.
