氏 名授 与 し た 学 位専攻分野の名称学 位 授 与 番 号学位授与の 日付学位授与の要件学 位 論 文 題 目Julianilbrahim博 士医 学博甲第 4503 号平成 24年 3月23日医歯薬学総合研究科社会環境生命科学専攻(学位規則第4粂第 1項該当)FrequencyofAntenatalCareVisitsandNeonatalMortalityinIndonesia(インドネシアにおける妊産婦健診の頻度と新生児死亡)論 文 辛 査 委 員 教授 荻野 景規 教授 官石 智 准教授 塚原 宏一学 位 論 文 内 容 の 要 旨Indonesiahasoneofthehighestneonatalmortalityratesintheworld.Weexaminedtherelationshipbetweenfrequencyofantenatalcarevisits,asawholeandineachtrimester,andneonatalmortalityinIndonesia.Dataon13,055singlebirthswereobtairxtdfromthefifthhkloI℃SiaDさrmgraphicI七athSurveycorductedin20061007.Weanalyzedtheassociationbetweenantenatalcarevisits(thetotalpregnancyandin･eachtrimester)andneonatalmortality.Weestimatedadjustedoddsratios(ORB)andtheir95%confidencehtervals(CIS)fortheassociationusingthegroupdthleastvisitsasareference,adjustingforreg10n,agegroups,maternaleducationlevels,andbirthorder.Pregnantwomenwhohadmoreantenatalcarevisitsexperiencedalowerriskofneonatalmortality.Inparticular,antenatalcarevisitsinthelasttrimesterwereofmorebenefit:theORsforneonatalmortality,againstthe0-1Visitgroup,were0.76(95%CI-0.45,I.29)inthe2visitsgroup,0.54(95%CI-0.33,0.87)inthe3visitsgroupand0,31(95% C1-0.17,0.57)inthe4visitsgroup.Although individua10Rswerenotsignificantintheassociationbetl鳩entOtalnulTt)erSOfantelutalcarevisitsardneorntalmortality,ORBdeclinedmarkedlywith7visitsormore.Thepresentstudyshowedarelationshipbetweenantenatalcarevisits,especiallyinthethirdtrimester,andareductioninneonatalmortality.TheseresultsmayprovideavaluablerecommendationforthecareofpregnantwomeninIndonesia.論 文 審 査 結 果 の 要 旨2006年から2007年にかけてインドネシアの第5回人口統計健康嗣査で､1,694の地域からそれぞれ25世帯を抽出し､34,227人の女性から質問調査を行い､看護師又は助産節の妊娠前健康管理訪問回数と出産後 28日以内の新生児の死亡との関係を解析した｡健康管理訪問時期を最初の3ケ月､2番目の3ケ月､7ケ月日から出産までの最終期間に分け､それぞれの期間での訪問回数と新生児死亡の関連性を､ロジスチック解析を行い､新生児死亡のオッズ比と95%信頼区間で表した｡最終期間に於いて､健康管理訪問回数の増加に伴う新生児死亡オッズ比の有意な減少が､地域､年齢､母親の教育､出生順位を調整しても認められた｡すなわち､7ケ月目から出産までの最終期間に看護師又は助産師の健康管理訪問回数が多いほど､新生児死亡が有意に減少することが判明した｡以上より､本研究はインドネシアにおける周産期医療に貢献する価値ある業練と認める｡よって､本研究者は博士(医学)の学位を得る資格があると静める｡
