This thesis aims to assess the current body of evidence and experiences with continuous flow-LVADs in the new era of advanced heart failure therapy. Furthermore, we investigate clinical outcomes, complications, and the impact of LVAD on end-organ function. In addition, an effort was made to predict these end-points in order to improve the selection criteria for LVAD therapy and current clinical practice

The EUROMACS-RHF risk score outperforms currently known risk scores for right heart failure and can be useful for tailored risk-based clinical assessment and management of patients with advanced heart failure evaluated for left ventricular assist device (LVAD) therapy. (this thesis)   2. The longer a patient is supported with an LVAD the more suspicious clinicians should be for mechanical device failure. (this thesis)   3. Acute  kidney  injury  following  LVAD  implantation  is  largely  the  consequence  of  the  deteriorating  condition  of  the  LVAD  recipient  prior  to  LVAD  implantation.  (this  thesis)  4. The impact of co-morbidities is more severe in older patients, and additional effort should be made to optimize their condition prior to LVAD implantation. (this thesis)   5. Nearly all LVAD patients suffer from the acquired von Willebrand syndrome. However, a definite association with bleeding events has yet to be determined. (this thesis)   6. The right ventricle remains the Achilles heel of LVAD therapy.   7. The “destination” should be clear for destination therapy LVAD patients.   8.
