The decrease of thyroxine binding g l o b u l i n (TBG) l e v e l s in serum during t o t a l f a s t i n g was documented e a r l i e r (1). In addi-t i o n , a decrease of t r i i o d o t h y r o n i n e (T^) but an increase of reverse T^ ( r T3' a n < ^ free f a t t y acids i n serum was shown (1) in 16 h o s p i t a l i z e d obese patients (8 male, 8 female) in the age of 15 to 46 (mean 27) years. In t h i s study, the influence of t o t a l f a s t i n g over a period of 31.3 _+ 9 days (mean +_ SD) on urinary iodine and t h y r o i d hormone excretion was i n v e s t i g a t e d . Methods : Chemical iodine determination was performed by the S a n d e l l - K o l t h o f f r e a c t i o n using a modified a ^ ^ a n a l y z e r tech-nique (Fa. Technicon, Frankfurt a.M.) f o r PB I determination. A Sephadex column technique was used f o r the radioimmunological measurement of T^ and T^ in urine as described previously (2). For s t a t i s t i c a l c a l c u l a t i o n s the paired t - t e s t was used. Results : Before f a s t i n g the excretion of both t h y r o i d hor-mones was diminished i n these obese pati e n t s as compared to the normales in our region (T.: 0.45 +_ 0.21 ug/d vs. 1.44 0.51, p<0.001; T : 0.83 + 0.37 vs. 1.70 + 0.40 ug/d) . - The T 4 excre-t i o n showed a s l i g h t increase during t o t a l f a s t i n g up to 0.72 +_ 0.52 ug/d (n.s.) at the 32nd day of s t a r v a t i o n . In contrast the T^ excretion was found to be reduced to 0.60 +_ 0.36 jig/d (p< 0.025) at the end of the f a s t i n g period. The e s s e n t i a l decrease, i . e . to 0.56 _+ 0.31 ug/d, occurred during the i n i t i a l 5 days, r e s u l t i n g i n a reduction of the T^/T^ r a t i o by 49 %. Before f a s t i n g the iodine excretion in obese pati e n t s was found to be in the range of c o n t r o l s in t h i s endemic iodine d e f i c i e n c y area (42.7 + 12.3 ug/d) . The excretion decreased r a p i d l y to 23.7 _+ 10.1 ug/d (p<0.01) during the i n i t i a l 5 days of t o t a l f a s t i n g , but decreased t h e r e a f t e r only s l i g h t l y to 18 +_ 10 ug/d at the end of the f a s t i n g period. Conclusions : 1) S u r p r i s i n g l y , the t h y r o i d hormone excretion seems to be diminished in obese patients before f a s t i n g . 2) The decrease of the T^ excretion induced by f a s t i n g was in p a r a l l e l to the f a l l of serum T^ l e v e l s . 3) The iodine excretion was impaired promptly during the f i r s t 5 days of t o t a l f a s t i n g , whereas the TSH l e v e l s did not r i s e (1). Thereafter, the iodine excretion remained constant at l e v e l s higher than observed in chronic severe iodine d e f i c i e n c y .
