The accidents and occupational illness are still found that can be caused by the lack of company’s awareness or employees about occupational and health safety. In order to avoid the cases of accident and occupational illness as well as potential that perhaps occur, it can be conducted by identifying and assessing of hazards which had never been done by CV X. This research aimed was to analyzed the hazard and assess the risk level as the cause of accident and occupational illness at the production section of CV X. The method used in this research was observational descriptive. The population of the research was all the 525 peoples in the production section with 53 peoples as the sample taken by simple random sampling technique. The results of the identification and assessment of risk level showed: 2 hazards were in high category; 5 hazards were in medium category; 32 hazards were in low category and 2 sources had no hazard. The company’s existing controls were engineering, administrative and personal protective equipment. The researcher suggestions for the company is to conduct risk management of hazards, so it can taken the control measurement promptly, especially for hazards that has medium and high risks
