Herein, we report a case involving a50-year-old man diagnosed with mantle cell lymphoma-blastoid variant who experienced spontaneous splenic rup-ture. The patient underwent splenectomy and recovered without incident. At first histological examination of thespleen suspected chronic lymphocytic leukemia. About one month after splenectomy multiple lymphadenopathydeveloped. A lymph node biopsy resulted mantle cell lymphoma-blastoid variant. He received R-CHOP (rituxi-mab, cyclophosphamide, doxorubicin, vincristine, and prednisolone) , hyper-CVAD/MA chemotherapy (hyper-fractionated cyclophosphamide, vincristine, doxorubicin and dexamethasone alternating with methotrexate andcytarabine), and ICE therapy (ifosfamide, carboplatin, and etoposide). However despite intensive chemotherapies,the disease became refractory, and the patient died 5 months after initiating chemotherapy. We discuss a case ofhigh-grade lymphoproliferative disease with spontaneous splenic rupture treated with splenectomy and chemo-therapy.Key Words: mantle cell lymphoma, blastoid variant, spontaneous splenic rupture緒 言マントル細胞リンパ腫は非ホジキンリンパ腫の中で予後不良の組織型として知られているが，その中の blastoid variantは急速な進行により予後絶対不良な亜型である．またリンパ増殖性疾患は脾腫がよくみられるが，非外傷性脾破裂は稀な合併症である．今回非外傷性脾破裂で発症したマントル細胞リンパ腫 blastoid variantを経験したので報告する．症 例患者：50歳，男性．主訴：表在リンパ節腫脹．既往歴：特記事項なし．：浅野千尋 〒276―8524 千葉県八千代市大和田新田 477―96 東京女子医科大学八千代医療センター血液内科E-mail: asano.chihiro@twmu.ac.jpdoi: 10.24488/jtwmu.87.6_170Copyright Ⓒ 2017 Society of Tokyo Women’s Medical University 東女医大誌 第 87 巻 第 6号頁 170～173 平成 29 年12月21―171―Fig.　1　Computed tomography scan showing splenic rupture and intraperitoneal bleedingFig.　2　Histological findings of the lymph nodeLarge abnormal lymphocytes spread in diffuse (Hematoxylin and eosin stain (A) ) and were positive for CD5 (B) and cyclin D1 (C).現病歴：当院入院 1日前の 17時頃突然の心窩部痛が出現した．症状が持続したため近医救急外来を受診し CTスキャンで脾破裂と診断され入院となった（Fig. 1）．翌日腹痛増強したため当院消化器外科に転院し，経過観察となっていた．当院入院 2日目腹痛増強，血圧低下あり，出血の増強と診断し，開腹脾臓摘出術を施行した．脾臓の病理所見では慢性リンパ性白血病疑いとされた．その後経過は順調で入院 16日目に退院となった．翌日血液内科を初診し，慢性リンパ性白血病として経過観察を行うこととなった．当科初診後約 2週間で頸部，腋窩，鼠径リンパ節腫脹が出現した．頸部リンパ節生検を施行し，細顆粒状の核クロマチンを有する大型の異型細胞が瀰漫性に増殖しており，免疫染色では CD5，cy-clin D1陽性でマントル細胞リンパ腫 blastoid vari-ant，mantle cell lymphoma international prognosticindex（MIPI）：intermediate riskと診断された（Fig.2）．染色体は正常核型であった．以前の脾臓の標本を再検討すると同様の細胞が認められた．化学療法目的に当科第 1回目入院となった．入院時現症：血圧，体温等バイタルに異常はなし．表在リンパ節では顎下 φ1.5 cm，2個，左腋窩 φ4 cm，左鼠径 3×8 cmを触知した． 眼球結膜に黄染なく，眼瞼結膜に貧血なし．胸腹部に異常所見はなかった．入院時検査所見：白血球 7,890/μl，Hb 10.8 g/dl，血小板 25.2万/μlと軽度貧血を認め，LDH 251 U/l，sIL-2R 5,350 U/lと上昇していた．臨 床 経 過：入 院 2日 目 に CHOP（cyclophos-phamide，doxorubicin，vincristine，and predniso-lone）療法 1コース目を施行し，腫瘍崩壊を考慮して22―172―Fig.　3　Clinical course and treatmentCHOP, cyclophosphamide, doxorubicin, vincristine, and prednisolone; Hyper-CVAD/MA, hyper-fractionated cyclophosphamide, vincristine, doxorubicin and dexamethasone alternat-ing with methotrexate and cytarabine; ICE, ifosfamide, carboplatin, and etoposide; sIL-2R, soluble interleukin-2 receptor.01000200030004000500060007000sIL-2R  U/mlCHOP RituximabSplenicRupture Hyper-CVAD/MA ICETable　1　Laboratory findings at the time of diagnosis＜CBC＞ ＜Chemistry＞ ＜Bone marrow＞RBC 386×104 /μl T-P 6.4 g/dl NCC 3.7×104 /μlHb 11.6 g/dl Alb 3.5 g/dl MgK 15.6 /μlHt 36.1 % AST 30 U/L G/E 4.71WBC 4,920 /μl ALT 24 U/L Atypical lymphocytes 41.5 %Neu 90.7 % LDH 257 U/L CD5＋ , CD19＋ , CD20＋Eo 2.8 % G-GTP 14 U/L 46,XY [20]Bas 0 % T-Bil 1.3 mg/dlLym 4.3 % BUN 15.4 mg/dlMom 2.2 % Cre 0.80 mg/dlPlt 34×104 /μl CRP 1.16 mg/dlsIL-2R 3,240 U/ml入院 5日目にリツキシマブを投与した．一旦リンパ節腫脹は改善傾向になったが，入院 15日目より発熱，リンパ節腫脹の増悪を認めた．骨髄抑制からの回復を確認し，入院 26日目に Hyper-CVAD/MA（hyperfractionated cyclophosphamide，vincristine，doxorubicin and dexamethasone alternating withmethotrexate and cytarabine）療法 1コース目を施行した．同時に投与していたリツキシマブはインフュージョンリアクションで中止となった．入院 52日目より Hyper-CVAD/MA療法 2コース目を施行した．入院 63日目より口蓋 桃，顎下，頤下，頸部，腋窩リンパ節腫脹の増大を認めた．入院 75日目よりHyper-CVAD/MA療法 3コース目を施行した．入院 101日目頃より頸部リンパ節腫脹の増大を認めるとともに sIL-2Rの上昇を認め，原疾患は Hyper-CVAD/MA療法に抵抗性と考え，同種造血幹細胞移植を予定して移植までの病勢コントロール目的でICE（ifosfamide，carboplatin，and etoposide）療法1コース目を施行した（Fig. 3）．入院 114日目に呼吸困難感が出現し，CTスキャンで舌骨上部のリンパ節腫大による気道閉塞があり気管切開を行った．気管切開後も胸腹水貯留による呼吸状態の悪化が進行し，入院 134日目永眠．考 察マントル細胞リンパ腫はリンパ濾胞のマントル層を構成する B細胞由来の腫瘍で，免疫組織学的には23―173―CD5，cyclin D1が陽性である．マントル細胞リンパ腫の組織学的亜型は aggressive variantsであるblastoid，pleomorphicタイプと古典型である smallcellタイプに分類される１）．blastoid variantは繊細な核クロマチンを有する N/C比の高い類円形細胞形態を呈している．一方 small cellタイプは小型から中型で細胞形態は均質で細胞質に乏しく変形した核を持ち，胚中心細胞に類似している．マントル細胞リンパ腫は非ホジキンリンパ腫の中では予後不良群で知られているが，治療への反応も悪く生存期間中央値は古典型の 35～70か月に比べて blastoid vari-antは 11か月と予後絶対不良である２）．造血器腫瘍において脾腫はよくみられる所見であるが，非外傷性脾破裂は稀な合併症である．Lunningらのリンパ増殖性腫瘍に合併した非外傷性脾破裂21例の報告では，急性リンパ性白血病 8例，慢性リンパ性白血病 3例，濾胞性リンパ腫 3例などとともに，マントル細胞リンパ腫 blastoid variantが報告されている３）．リンパ増殖性腫瘍では巨大脾腫をきたす有毛細胞白血病や慢性リンパ性白血病などが知られているが非外傷性脾破裂はかなり稀な合併症であると考えられる．リンパ増殖性疾患における非外傷性脾破裂は脾臓の大きさだけではなく増殖速度の速さが大きく関与していると言われており，その点から考 慮 す る と aggressive variantsで あ る blastoidvariantはリスクが高いと考えられる．blastoid vari-antの非外傷性脾破裂の報告は 9例みられ，本症例は我が国では 2例目の症例である４）～１２）．マントル細胞リンパ腫の予後予測モデルとしてMIPIが発表されており，年齢，全身状態（PS），LDH，WBCにより計算され，予後が 3群に層別化されている１３）．本症例は急激な進行にも関わらず，MIPIでは intermediate risk群に分類された．当初病理では脾臓標本が，CD5，CD20陽性から慢性リンパ性白血病と診断されていたが，非外傷性脾破裂を起こしたことから急速な進行を特徴とするマントル細胞リンパ腫 blastoid variantの可能性を検討すべきであったと考えられる．結 論非外傷性脾破裂を起こすリンパ増殖性疾患は稀だが，マントル細胞リンパ腫 blastoid variantなど高悪性度の疾患も考慮して治療へ反映させることが必要と考えられる．開示すべき利益相反はない．文 献1）Tiemann M, Schrader C, Klapper W et al: Histo-pathology, cell proliferation, indices and clinical out-come in 304 patients with mantle cell lymphoma(MCL): a clinicopathological study from EuropeanMCL Network.
