Technical difficulties and experience of the operators are contributors to complication apart from patient’s factors such as vascular anatomy, bleeding coagulopathy and body mass index.  Guided procedures are advantageous since the complications can be seen promptly and the endovascular treatment can be done immediately.    We presenting a case of an elderly renal failure patient had a complicated left permanent catheter insertion. The left brachiocephalic vein perforation was immediately and successfully bailed out by applying endovascular balloon tamponade. Risk of complications, venoplasty procedure and imaging follow up were discussed.  Abstract
