In our de-partment, during the repair of VSPs with lesions in the left anterior descending artery area, infarct lesions arepre-operatively detected with ECG synchronized contrast computed tomography, and the scope of approach andexclusion is determined. Furthermore, in order to prevent complications of residual shunt, a double patch exclu-sion technique (DPET) is used in combination to preserve left ventriclar function.Materials: We identified two consecutive VSP patients who underwent this procedure from September toDecember 2015. There was an average of five days between AMI and the onset of VSP, and an average of oneday between onset of VSP and surgery.Results: All cases were extubated on the day after surgery and intra-aortic balloon pump assistance was alsowithdrawn. Without perioperative complications, patients were able to leave the intensive care unit, on average,6.5 days post-operation. anterior descending artery.比較して有意に成績が良かったと報告し６），Caimmiらは double patch technique を用いた VSP 閉鎖の30 日死亡率は 18.8 %で，術後残存シャントは認めなかったと良好な成績を報告している７）．しかしながら，稀な疾患である VSP に対するアプローチや手術手技に確立された見解は得られていない．京都第二赤十字病院では 2015 年 9 月から左前下行枝（LAD）領域に責任病変のある VSP に対して，術前に心電図同期 CT で梗塞巣を把握し，アプローチや exclusionの範囲，パッチのサイズを決定している．さらに残存シャントを確実に防止するために double patch法と infarct exclusion 法を併用する double patchexclusion technique（DPET）を行い良好な結果を得たので，文献的考察を加えて報告する．対象と方法1．対象2015 年 9 月から 12 月までに本術式を施行したVSP 連続 2 例を対象とした（Table 1）．症例 1：71 歳，男性．7 日前に AMI 発症．VSP直径（φ）12 mm，責任病変 LAD#7-99 %狭窄 delay，肺体血流比（Qp/Qs）3.6，肺動脈楔入圧（PCWP）22mmHg．人工呼吸管理下に翌日緊急手術施行．手術時間 4 時間 57 分で，残存シャントなし．術翌日抜管し，経過良好で術後 18 日目に独歩退院．症例 2：78 歳，女性．3 日前に AMI 発症．VSPφ18 mm，責任病変 LAD#7-total，Qp/Qs=2.6，PCWP20 mmHg．大動脈内バルーンパンピング（IABP）下に翌日緊急手術施行．手術時間 4 時間 44 分で，残存シャントなし．術翌日抜管し，一般病棟で心臓リハビリを行い術後 49 日目にリハビリ転院．2．手術テクニック術前に心電図同期造影 CT を行い，梗塞巣範囲を同定して手術戦略を立てた（Fig. 1）．胸骨正中切開にてアプローチし，上行大動脈送血，上下大静脈脱血にて体外循環を確立した．心拍動下に梗塞心筋である左室前壁を切開．用手的に正常心筋と梗塞心筋との境界を把握した後に，大動脈基部より順行性に心筋保護液を投与し心停止とした．VSP 周囲の脆弱な中隔心筋を切除した．ウシ心膜 patch（10×15 cm）を直径 4.5～5.5 cm の正円形にトリミング．8 針マットレスで均等に中隔へ圧着するために皮膚ペンでマーキングしたものを 2 枚作成した左室側の 2nd patchは infarct exclusion 用の patch を結合させたもの（Fig. 2A）．作成した右室側の 1st patch に同心円状39―119―Fig.　2　Details of DPET in acute anterior VSPA: Two patches were ポリプロピレン糸を 8 針マットレスで通し，その針糸を VSP 周囲の比較的健常な中隔心筋に右室側から左室側へ貫通させた．また円形 patch 上縁の 3 針は右室自由壁へ刺出した．1st patch は VSPを介して右室に導き（Fig. 2B），心室中隔を貫通させた針糸のセットを 2nd patch に通して，patch の間に は GRF glue（Cardial, Technopole, Sainte-tienne,France）もしくは Bio glue（Cryolife Inc., Kennesaw,Georgia, USA）を注入した．double patch 完成後（Fig. 2C），infarct exclusion 用の patch を円形 patchの下縁から時計回りに mattress suture で糸掛けを行った．patch SVG: great saphenous vein.宜 patch をトリミングして梗塞部と double patchを exclusion した（Fig. 2D）．左室切開線は 3-0 ポリプロピレン糸を使用し，フェルトサンドウィッチ法にて 2 重に縫合閉鎖した．最後に LAD へバイパス吻合を追加し手術を終了した（Fig. 2E）．結 果2 例ともに梗塞心筋側の左室前壁アプローチ法で行い，coronary artery bypass grafting（CABG）を追加した．平均手術時間は 290 分，平均大動脈遮断時間は 115 分，平均体外循環時間は 192 分であった．全例とも術翌日に抜管し，IABP 補助も術後 17 時間で離脱した．周術期合併症もなく平均 6.5 日で ICUを退出した．術後心エコー検査では残存シャントは認めず，冠動脈 CT ではバイパスも開存していた．全例心機能低下による心不全症状も認めず，その他の合併症もなく退院が可能であった（Table 2）．考 察急性期 VSP の治療は，患者の循環動態の不安定さ，VSP 周囲の心筋の脆弱さから手術に難渋することが多い．今回の報告では 1 例は術前に IABP が挿入され，1 例は人工呼吸管理が必要なほど呼吸循環動態が悪化しており，急性期での手術が必要と判断された．今回，我々は，非梗塞心筋の障害を最小限にするために梗塞心筋側である左室前壁からアプローチした．また，残存シャントを確実に予防し，さらに術後の心機能を保持する目的で，doublepatch exclusion technique により，全例 VSP を確実に閉鎖し良好な経過を得ることが出来た．Doublepatch technique と infarct exclusion technique の両方の利点を，比較的容易に同時に達成できる本術式は有用で効果的な術式と考えられた．VSP に対する外科治療は 1957 年に Cooley らにより初めて報告され８），その後さまざまな改良や進歩が加えられ，1990 年に Komeda らにより報告されたinfarct exclusion 法によりその手術成績は大きく改善された９）１０）．本術式の特徴は正常心筋にパッチを縫着することで，梗塞心筋を exclusion し，過剰な心筋切除を行わず，結果的に左室の収縮拡張機能を保持でき，また，切開部からの出血低減，術後心室瘤を予防できるという点である．しかし，パッチ縫着時の左室流出路や左室後壁への運針は，術野がかなり深く，また後乳頭筋が存在することから，かなりの工夫と熟練を要する．その結果術後シャントが残存し再手術が必要な症例や，心不全から死亡に至る症例も多くある１１）１２）．一方で VSP は急性心筋梗塞後の稀な合併症であり，心臓外科医が 1 年間で経験できる症例数は数えるほどしかないため，Asai は VSP修復術の手技は簡便である方が良いと報告している１３）．また，近年 VSP を 2 枚のパッチで挟み込むように閉鎖する double patch 法の良好な成績が多く報告されている１３）．VSP に対する double patch 法の利点は，片側の心室内腔のみからアプローチできる点や手技が簡便で容易に行える点である．この方法は 2枚のパッチを左右両心室から梗塞中隔心筋を挟むように均等に 8～10 針の mattress 結節縫合を行うが，それにより左室圧が拡散され，圧負荷による縫合離解を軽減することができる１４）．さらに，パッチ間にはGRF glue や Bio glue などの生体糊を補填することでさらに強固に残存シャントを予防できる１５）１６）．また長期的には double patch 法は心室中隔の壁運動異常を軽減するとも報告されている７）．しかし LAD領域の心筋梗塞による VSP に対して右室アプローチによる double patch 法では術後に右室切開による右心不全が問題となることもある１７）１８）．当科でも今回の術式を用いる以前は右室アプローチによるdouble patch 法のみで VSP 手術を施行していたが，経験した 6 例中 2 例が術後右心不全を伴う低心拍出量症候群（LOS）により在院死している．そこで今回の報告では，術式を左室アプローチで行い，VSP の閉鎖のため double patch 法に加えてさらに exclusion 法を追加した．この目的は，①doublepatch に直接左室圧が加わることを避けることで残41―121―存シャントをより確実に予防すること，②左室切開線からの出血を制御し，術後の左室瘤や左室破裂を予防することで心機能を維持すること，である．同様の観点から double patch 法に infarct exclusion法を別個に追加した報告では，残存シャント予防と術後心機能保持に有用な術式だとしている１９）．本術式は 2nd patch と infarct exclusion 用のパッチを一連のパッチにすることで，最も視野が深く縫合困難な左室後壁側での exclusion 縫合が省略できるため，簡便に double patch 法と infarct exclusion法の両術式を行える有用な方法と思われた．また左室内腔が小さくなりすぎないように，術前心電図同期 CT を行い梗塞範囲を同定してからパッチのサイズやアプローチ部位を決定した．これによりジャストサイズのパッチで infarct exclusion することができたため，遠隔期の左室リモデリングを予防し左心機能を保持するのに有用であったと思われた．結
