PRESIDENTIAL LECTUREBuilding Bridges: From Research on Sexual Problems to the Promotion ofSexual Health and RightsPedro NobreFaculdade de Psicologia e Ciências da Educação, Universidade do Porto, Porto, PortugalIn this lecture I will review 25 years of research on psy-chosocial determinants of sexual function and dysfunc-tion in men and women, with particular emphasis inthe role of cognitive and emotional factors. Studiesusing survey methods and cross-sectional designs, aswell as longitudinal studies and experimental studiesconducted in the laboratory will be reviewed systemat-ically, and a summary of the main findings for theunderstanding of sexual dysfunction and implicationsfor the treatment and prevention of sexual problemswill be presented.Moreover, I will build a bridge between the preven-tion of sexual problems and the promotion of sexualhealth. However, the framing of sex education andsexual health programmes continues to be one of avoidingdanger, avoiding death, and avoiding disease rather thanseeking pleasure, enjoying and affirming our bodies, andarticulating desires. In fact, pornography, which rarelyshows safer sex but has immense erotic appeal, is nowone, if not the key channels for sex education globally.Action: The omission of pleasure in sex educationcurricula has a damaging effect on sexual health andwellbeing by failing to recognise diverse sexualities, andperpetuating the myth that women are not sexualbeings. On the other hand, people with a more positiveview of their own sexuality are more likely to practicesafer sex, use contraception consistently, have highersexual self-esteem and therefore be more sexually assert-ive and confident. Despite evidence that attitudes aboutsexuality influence safer-sex practices and agency, fewevaluations of sex education and sexual health interven-tions consider these aspects of people’s sexualexperiences. The recent massive increase in access toerotic media globally, with a very limited view of pleas-ure, gender, race and sexuality, makes the need formore holistic discussions of sexuality and sexual healthmore urgent. We need to advocate and undertake more‘pleasure audits’ of organisations who aspire to improvesexual health and well being, and share these forresponsibility for global well-being and health.Conclusion: We need to champion pleasure educa-tion as part of safer sex education and making eroticasafe. The two worlds of pleasure and prevention arestrange bedfellows, with vastly different epistemologies,but with much to learn from each other.
