Nursing role in the limitation of therapeutic effort – LTE – in pediatric patient.

Abstract

Trabajo fin de grado en EnfermeríaObjetivo: Analizar la evidencia disponible sobre la limitación del esfuerzo terapéutico en el paciente pediátrico y el rol enfermero. Metodología: Se realizó una revisión narrativa a través de la búsqueda bibliográfica en las bases de datos de Ciencias de la Salud: Cinahl, Cuiden, SciELO España, Lilacs, Dialnet y Pubmed, haciendo uso del tesauro MesH en esta última. Los criterios de inclusión utilizados fueron: artículos con resumen y disposición de texto completo gratuito, publicados en un intervalo de tiempo 2009-2020, en idioma inglés y/o español y acotados a la edad 0-18 años. Resultados: Fueron seleccionados 110 artículos que, en un principio, tras la lectura de título y resumen, se correspondían con la temática de estudio. Al realizar una lectura exhaustiva de los mismos, se seleccionaron 28 artículos los cuales se aproximan a los objetivos del trabajo. La información obtenida se organizó en 8 categorías: LET, Rol enfermero en LET, Ética en LET, Comunicación al final de la vida, Toma de decisiones al final de la vida, Familia, Afrontamiento de la muerte, CPP. Conclusiones: La decisión de LET en el paciente pediátrico es un tema de gran controversia ética y moral. El proceso de toma de decisiones ha de ser llevado a cabo de forma multidisciplinar, incluyendo en todo momento a la familia del paciente. A pesar de ello, la literatura refleja la escasa participación enfermera en la misma, resultando por ello de gran relevancia promover su reconocimiento.Objetive: To review evidence about limitation of therapeutic effort in the pediatric patient and the nursing´s role at this practice. Methodology: A narrative review has been made. To do so, we carried out a bibliographic research in the health sciences’ databases: Cinahl, Cuiden, SciELO España, Lilacs, Dialnet y Pubmed, using the thesaurus MesH in the last one. The inclusion criteria were as follow: abstract and free available full-text articles, published between 2009- 2020, in English or Spanish and enclosed in 0-18 years old. Results: Firstly, after reading article’s title and summaries, we selected 110 articles according to the topic´s study. Then, after doing and exhaustive reading and research of each articles, we selected the final 28 articles. With all this information information that it was obtained, 8 categories were created: Therapeutic limitation effort, Nursing role in therapeutic limitation effort, Ethic, Communication in end of life, Decision making an end of life, Family, Dead´s confrontation and Pediatric palliative care. Conclusions: The decision of therapeutic limitation effort in the pediatric patient is a very etic and moral controverted topic. The decision-making must be carried out by a multidisciplinary team, including the family. Despite of this, the literature indicates a shortage nurse’s participation in it.
