Inver-sion sprains cause pain, swelling, stiffness, instability, andpredisposition to reinjury. Untreated, these become riskfactors for reinjury. This randomized clinical trial comparedankle manipulation (group 1) versus muscle energy techniquemobilization (MET) (group 2) in the treatment of mild tomoderate, chronic recurrent ankle inversion sprain, or ankleinstability syndrome (AIS).Methods: Seventy-one AIS candidates, ages 18 to 50, wereexamined and 40 were included. Twenty were randomized tomanipulation and 20 to MET. The primary outcome measurewas the one leg standing test (OLST); secondary measuresincluded the numerical (pain) rating scale-101 (NRS-101),inclinometer, the functional evaluation scale (FES), and theshort-form McGill pain questionnaire (SFMPQ). Six treat-ments over 3 weeks were delivered. Outcome measures werecollected at baseline and at the 4th and the follow-up 6thvisit.Results: No difference was detected between groups. Post-hoc power suggested that N D 40 was too small to detectdifferences. Resources did not allow for a blind assessor.Therefore, no efficacy extrapolation between groups shouldbe made. However, Kolmogorov–Smirnov tests demonstratednormal data. Consequently, paired t tests demonstrated signif-icant and clinically meaningful changes for the OLST, NRS-101, SFMPQ, dorsiflexion, and FES at p  .05. The OLSTwas ½10 seconds, which suggests a centrally mediated posi-tive neurological effect on the function of the locomotivesystem.Conclusions: Ankle manipulation and mobilization signifi-cantly increased balance, range of motion, and function. Bothdecreased pain, appearing beneficial in short-term AIS treat-ment. Studies were limited to thosepublished in English and in peer-reviewed journals or confer-ence proceedings between January 1980 and March 2009. Allstudy designs were considered except personal narratives orreviews. Retrieved articles that met the inclusion criteria wererated for quality by using the Downs and Black checklist. Anannotated bibliography was also written of each retrievedstudy.Chiropractic Care of a Student Athlete with Lumbar Disc Herniation, Sacroiliac JointDysfunction, Spina Bifida Occulta, and Clasp-Knife SyndromeKimberly Keene, Palmer College of Chiropractic FloridaObjective: Chiropractors frequently treat lumbar discpathology in practice, but typically in adult patients. This caseinvolves a 16-year-old football player diagnosed with an L5disc herniation in association with sacroiliac joint dysfunc-tion, spina bifida occulta, and clasp-knife syndrome.Results: Eight articles met the inclusion criteria of thisreview in the form of one case series, one case study, onesurvey, two randomized controlled trials, one randomizedpatient and observer-blinded cross-over trial, one single-blindcross-study design, and one self-reported impairment ques-tionnaire. Their quality scores ranged from 5 to 22 out of 27.Conclusion: Results of the eight retrieved studies indicatedthat chiropractic care showed improvements in subjectivemeasures and, to a lesser degree, objective measures, noneof which were statistically significant. It is evident that someasthmatic patients may benefit from this treatment approach;however, at this time, the evidence suggests chiropractic careshould be used as an adjunct, not a replacement, to traditionalmedical therapy.Clinical Features: A 16-year-old male high school footballplayer with a history of chronic low back pain and multipletraumas sought care for his chronic low back pain.
