Thesuccess of the CIHR will be determined by the degree towhich Canada is able to provide focus and the resourcesto tackle the critical health issues affecting Canadians.The health of mothers, children and youth is of centralimportance for all Canadians. Women of childbearing age(5.6 million people), and children and youth (eight mil-lion) comprised 45% of the population in 1998 and useda significant portion of Canadian health care resources.The ability to reproduce, to support a safe and healthypregnancy with normal fetal development and birth, andto allow children and youth to grow and develop nor-mally, free from neglect, disease and pain, are the funda-mental goals of Canadian society.Critical health issues affecting the periods of precon-ception, pregnancy, fetal and newborn development, andgrowth and development throughout childhood and ado-lescence are naturally linked to each other other throughtheir interdependence and through existing organizationswithin the Canadian research community. The creation ofa Canadian research institute concerned with the healthof mothers, children and youth would provide an oppor-tunity to focus and to build on existing national andinternational strengths. For this proposed institute, theworking name, The Canadian Health Research Institutefor Mother, Child and Youth (CHRIMCY) has beenadopted. The proposed primary goals of CHRIMCY are toinitiate, support and disseminate excellence in peer-reviewed research encompassing the critical stages of life,including preconception, pregnancy, fetal and newborndevelopment, and growth and development throughoutchildhood and adolescence. These primary goals definethe nature of research that would be supported by theproposed institute, and clearly distinguish this institutefrom others that may form within the CIHR. Researchwould be conducted in the fields of basic biomedical sci-ence, clinical and applied health, health services andhealth care systems, and population health, with a par-ticular focus on the determinants of health.WHY A RESEARCH INSTITUTE FOR MOTHERS,CHILDREN AND YOUTH?The proposed main objective of CHRIMCY is to rein-force individual investigator initiatives by aiding in theformation of networks of research teams that would crossdisciplines, institutions, regions and the country. CHRIMCY would havethe unique mandate and framework to identify researchthemes and to harness research expertise across thecountry, and to focus on priority issues affecting thehealth of mothers, children and youth.By fostering networks across Canada and by promot-ing the creation of pools of biological samples and data,CHRIMCY will be key in addressing the lack of criticalmass, in terms of size and diversity of patient cohorts,that often plague epidemiological and clinical trials inthese population groups. This, in turn, will make indi-vidual investigators and their networks more attractiveto funding from private foundations, and provincial,national and international funding agencies, and forpartnerships with industry. By providing a nationalframework for setting priorities and for identifying andfocusing resources and energies that can be applied tothose issues, CHRIMCY will also help eliminate unneces-sary duplication of research infrastructure, freeing addi-tional funds to support a greater depth and breadth ofstudies.BUILDING ON STRENGTHThe maternal-child health research mission in Canadacurrently involved over 30 research themes within sixmajor axes. In 1998, these efforts represented over 700investigators and over $125 million in internal and exter-nal research support. This extensive infrastructure con-stitutes a core of existing strength for the development ofan institute for “mothers, children and youth” within theCIHR.The presence of national professional bodies linked touniversity departments that have a clear mandate to sup-port the development of their fields through the acquisi-tion and application of new knowledge has been a criticalcomponent of the developmental phase of CHRIMCY. Theactive involvement of the many researchers, partners andpotential partners who have come together during this de-velopment phase of CHRIMCY demonstrates the impor-tance that is being placed on the CIHR initiative. While notyet representing all the disciplines potentially involved inthe area of maternal, child and youth research, the iden-tified organizations (Table 1) comprise thousands ofdedicated professionals in the field of maternal, childand youth health.These organizations are currently working to integrateand disseminate information to consumers, to providequality information based on evidence and to stimulatethe identification of new issues of concern to Canadians.In January 1999, the American Academy of Pediatrics(AAP) launched the Center for Child Health Research,which has much in common with the stated mission goalsof CHRIMCY, and which may be an important interna-tional partner.The commitment of the federal government to addressthe health of children and youth, as mentioned in the recentThrone Speech, is important to the success of this pro-posed institute. The proposed institute would work in part-nership with the government to support this commitmentwith new knowledge and new methods for ensuring the dis-semination and application of the new knowledge.CHRIMCY WEBSITEThe Internet is an outstanding tool for communicationand will be used extensively in the process of establishingCHRIMCY.
