We report the case of a 49-year-old patient, with recent onset of fatigue, who was diagnosed with atrial fibrillation, which was cardioverted electrically. A diagnosis of right atrial aneurysm, with no other cardiac pathology, was made. An aneurysmectomy was performed. Both surgery and postoperative recovery were uneventful

Pode ser deorigem congénita ou adquirida, considerando-sede origem congénita primária na ausência defactores etiológicos predisponentes, tais comodoença valvular tricúspide, hipertensão arterialpulmonar, ou alterações inflamatórias adquiridasdo miocárdio. Com excepção de dois de novecasos que conseguimos recolher da literatura,todos os pacientes apresentavam taquiarritmiasupraventricular. A ressecção cirúrgica doaneurisma da aurícula direita, de baixo risco, estáindicada em todos os pacientes, mesmo nosassintomáticos, pelas possíveis complicações quese lhes podem associar, tais como taquiarritmiassupraventriculares, formação de trombos e riscode embolia pulmonar, e ruptura do aneurismacom tamponamento cardíaco.INTRODUCTIONAtrial aneurysms are a rare form of cardiacaneurysm, and those of the right atrium are evenless common. They may be congenital oracquired; they are considered to be of primarycongenital origin in the absence of otherpredisposing etiological factors, such as tricuspidvalve disease, pulmonary hypertension, oracquired inflammatory changes in themyocardium. In seven of the nine cases we foundin the literature, the patients presentedsupraventricular tachyarrhythmia. Surgicalresection of right atrial aneurysms is a low-riskprocedure and is indicated in all patients, eventhose who are asymptomatic, given thecomplications that may arise, such assupraventricular tachyarrhythmia, thrombusformation, and the risk of pulmonary embolism orrupture of the aneurysm with cardiac tamponade.1050Rev Port CardiolVol. 26 Outubro 07 / October 07CASO CLÍNICOHomem de 49 anos de idade que recorreu àconsulta de cardiologia por início recente decansaço após exercício habitual. O ECG efectuadona altura demonstrou uma fibrilhação auricularque foi cardiovertida electricamente, ficando em ritmo sinusal desde então. Sem antecedentespatológicos relevantes. Subsequentemente foisujeito a avaliação cardiológica. Ao exame físico, apressão arterial sistémica (130/80 mmHg), e aauscultação cardíaca e pulmonar eram normais.Não se observou distensão venosa jugular. O RXdo tórax era normal, tal como o ECG, com ritmosinusal de cerca de 90 bpm. O ecocardiogramatranstorácico (ETT) revelou a existência de umadilatação “estranha” na aurícula direita, sem ano-malias da válvula tricúspide. A RMN demonstroua existência de um prolongamento aneurismáticoem continuidade com a parede livre da aurículadireita (5.2x5.8x5.2 cm), comunicando com acavidade auricular por um orifício de cerca de 1.7cm. O aneurisma prolongava-se ao longo daaurícula direita, de que estava separado por umfino septo, até ao ventrículo direito, a que causavaligeira compressão (Fig 1). CASE REPORTA 49-year-old man was referred for cardiologyconsultation for recent onset of fatigue followingnormal exertion. The ECG revealed atrialfibrillation, which was cardioverted electrically,and the patient has remained in sinus rhythm. He had no relevant history of disease. Hesubsequently underwent cardiological assess-ment. Physical examination showed systemicblood pressure of 130/80 mmHg and normalcardiac and pulmonary auscultation, and nojugular venous distension was observed. Thechest X-ray was normal, as was the ECG, withsinus rhythm of around 90 bpm. Transthoracicechocardiography revealed an abnormaldilatation in the right atrium, with no tricuspidvalve anomalies. Magnetic resonance imaging(MRI) showed an aneurysm, measuring5.2x5.8x5.2 cm, continuous with the right atrialfree wall, communicating with the atrium throughan orifice of around 1.7 cm. The aneurysmextended all along the right atrium, from which it was separated by a thin septum, up to the right ventricle, where it was causing slightcompression (Fig. 1). Afonso et alRev Port Cardiol 2007; 26: 1049-52  Tendo em conta a história natural, espe-cialmente a ocorrência frequente de arritmias,optou-se pelo tratamento cirúrgico. Sob CEC,procedeu-se à auriculotomia direita. A inspecçãoda anatomia revelou o volumoso aneurisma, compresença de miocárdio auricular a separar aaurícula da zona aneurismatica. Procedeu-se àressecção do aneurisma e reconstituição daparede auricular por aproximação directa dosbordos do colo do aneurisma. A operação e o período pós-operatório decor-reram sem incidentes e o doente teve alta no 5ºdia. Mantém-se bem e assintomático ao fim de 6meses, pelo que se considera curado.DISCUSSÃOO aneurisma da aurícula direita é umasituação extremamente rara, de etiologiadesconhecida, mas provavelmente de origemcongénita, e pode ocorrer em diferentes gruposetários, desde neonatos até adultos(1,2). Tem sidodescrito de diversas formas, desde aneurisma adivertículo ou dilatação idiopática. Baily et al.(3)foram os primeiros a descrever esta situação eMorrow e Behrendt(4) os primeiros a relatar aressecção de um aneurisma auricular direito. Éligeiramente mais frequente no homem que namulher(5,4). Os pacientes podem apresentar-seassintomáticos(2,5), mas frequentemente têmarritmias supraventriculares e/ou sinais deinsuficiência cardíaca(4,5) e o diagnóstico égeralmente despoletado pela arritmia auricular. Aformação de um trombo mural na aurícula direita,que constitui um risco potencial de emboliapulmonar, também está descrita(6).O aneurisma da aurícula direita pode estarassociado a outras malformações cardíacascongénitas, tais como defeitos dos septosauricular e ventricular e a divertículos do seiocoronário(7). As arritmias auriculares serãoprovavelmente secundárias à dilatação auriculare desorientação estrutural das fibras do sistemaespecializado de condução(8). A etiopatogeniadeste defeito é bastante desconhecida, masdeverá existir um defeito congénito da estruturada parede auricular, promotor das dilatações.Estão descritas alterações degenerativaslipomatosas e redução dos elementos muscularesna parede aneurismatica(1).O Ecocardiograma é o método preferido paraBearing in mind the natural history of suchaneurysms, particularly the frequent occurrenceof arrhythmias, surgical treatment was decidedupon. An atriotomy was performed withextracorporeal circulation (ECC). Anatomicalinspection revealed a large aneurysm, separatedfrom the atrium by atrial myocardium. Ananeurysmectomy was performed and the atrialwall reconstructed by directly joining the bordersof the neck of the aneurysm. Both surgery and postoperative recovery wereuneventful and the patient was discharged on thefifth day. He remains well and asymptomatic aftersix months and is therefore considered cured.DISCUSSIONRight atrial aneurysms are extremely rare;their etiology is unknown but they are probablycongenital, and can occur in all age-groups, fromnewborns to adults(1, 2). Various forms have beenreported, from aneurysm to diverticulum oridiopathic dilatation. Bailey et al.(3) first describedthis entity, while Morrow and Behrendt(4) were thefirst to report the resection of a right atrialaneurysm. They are slightly more common in menthan in women(5,4). Patients may be asymptomatic(2,5),but often have supraventricular arrhythmiasand/or signs of heart failure(4,5), with diagnosisgenerally being prompted by atrial arrhythmia.Thrombus formation in the right atrial wall, whichconstitutes a risk for pulmonary embolism, hasalso been described(6).Right atrial aneurysms can be associated withother congenital cardiac malformations, such asatrial or ventricular septal defects or diverticulaof the coronary sinus(7). Atrial arrhythmias areprobably secondary to dilatation and structuraldisarray of the fibers of the specializedconduction system(8) . The etiopathology of thisdefect is unclear, but there is probably acongenital defect in the atrial wall structure thatfavors dilatation. Lipomatous degeneration andreduction of muscular elements in theaneurysmal wall have been described(1).Echocardiography is the preferred techniquefor diagnosis and exclusion of associatedconditions. Distinguishing between atrialaneurysms and tumors or adjacent structures canbe difficult(6). Cardiomegaly is a frequent findingon chest X-ray. A cardiomegalia é um achado frequenteno RX do toráx. O ECG pode ser normal oumostrar arritmias supraventriculares. A TAC, aRMN do tórax e a angiocardiografia são úteis parao diagnóstico definitivo e para a caracterização aextensão(5,6).A análise da literatura não permite estabelecero prognóstico desta anormalia sem intervençãocirúrgica, nem o risco de recorrência da dilataçãoauricular e arritmias após a cirurgia. No entanto,os pacientes sintomáticos respondem bem àcirurgia(4,6) e o controle das arritmias tem sido bemsucedido com a excisão cirúrgica do aneurisma.Uma vez que este procedimento está associado aum baixo risco operatório, a ressecção deverá sertambém opção para os doentes assintomáticos,atendendo ao risco de complicações acimadescritas. A cirurgia é feita sob CEC, mas quando éisolado a ressecção do aneurisma pode ser feitasem CEC, com o uso de clamps vasculares(8). Emqualquer caso, dever-se-á proceder à ressecçãodo aneurisma e plastia de redução da aurícula. Seum trombo estiver presente no interior do sacoaneurismático, a artéria pulmonar poderá serocluida temporariamente a fim de prevenir aembolização, enquanto se procede à ressecção(4).A conservação do apêndice auricular é útil napreservação do factor natriurético auricular epara canulação, caso seja necessário futuraintervenção cirúrgica cardíaca(8).supraventricular arrhythmias. Chest CT and MRIand angiocardiography are useful for definitivediagnosis and characterization of the extent of theaneurysm (5,6).An analysis of the literature sheds little lighton the prognosis of these anomalies withoutsurgical intervention, nor on the risk ofrecurrence of atrial dilatation and arrhythmiasafter surgery. Nevertheless, symptomatic patientsrespond well to surgery(4,6) and arrhythmias havebeen successfully controlled following surgicalresection of the aneurysm. Since operative riskwith this procedure is low, resection should alsobe the option for asymptomatic patients in view ofthe risk of the complications mentioned above. The surgery is usually performed with ECC,but aneurysmectomy alone is also possiblewithout ECC with vascular clamping(8). In anyevent, the aneurysm should be resected andreduction atrioplasty performed. If a thrombus isfound inside the aneurysmal sac, the pulmonaryartery can be temporarily occluded to preventembolization during resection(4). Preservation ofthe atrial appendage is useful for maintainingatrial natriuretic peptide levels and forcatheterization should further cardiac surgery berequired in the future(8).Pedido de separatas para:Address for reprints:MANUEL J.
