This invasive procedure isconsidered safe, although it is not free of com-plications, such as the development of coro-nary fistulas. In our population, three hearttransplant patients developed coronary fistu-las to the right ventricle.The first case is of a 51-year-old male whounderwent heart transplantation in September2006 for end-stage familial dilated cardiomy-opathy. Durante o seu seguimento foram rea-lizadas 1617 biópsias do endomiocárdio(BEM). A biópsia é o método de primeira linhapara o diagnóstico de rejeição do enxerto. Esteprocedimento invasivo embora seguro, não éisento de complicações. Dentro destas desta-ca-se o risco de desenvolvimento de fístulascoronárias. Na nossa população, três dos doen-tes transplantados desenvolveram fístulas coro-nárias para o ventrículo direito. O segundo caso é relativo a um doente de43 anos, sexo masculino, transplantado emNovembro de 2008 por miocardiopatia dila-tada familiar. Após ter sido submetido a 11BEM, por via femoral direita, com colheita de23 fragmentos, um ano depois do transplante,é submetido a angiografia coronária. Namesma foi identificada imagem sugestiva defístula da descendente anterior média para oventrículo direito (Figura  nº 3).O terceiro caso trata-se de um doente de 53anos, sexo masculino, transplantado em Maiode 2009 por miocardiopatia dilatada idiopática.Durante o primeiro ano, foram realizadas 10BEM, todas por via femoral direita, comobtenção de 23 fragmentos. Em Maio de 2010é submetido a angiografia coronária, onde éidentificada uma fístula de reduzida dimen-sões, com origem no ramo ventricular direito dacoronária direita e drenagem para o ventrículodireito (Figura nº 4).Nenhum dos doentes desenvolveu sinto-mas clínicos ou apresentou repercussões he-modinâmicas. Os ecocardiogramas pós-BEMtambém não revelaram sinais de sobrecargaThe second case is of a 43-year-old maletransplanted in November 2008 for familialdilated cardiomyopathy. During the first year,11 EMBs were performed, via the rightfemoral vein, and a total of 23 fragmentsobtained. The coronary angiogram revealed animage suggestive of fistula between the anteri-or descending artery and the right ventricle(Figure 3).The last case is of a 53-year-old malepatient, transplanted in May 2009 for idio-pathic dilated cardiomyopathy. During thefirst year, he underwent 10 EMBs, all via theright femoral vein, a total of 23 fragmentsbeing obtained. In May 2010 coronary angiog-raphy was performed during which a small fis-tula was identified originating in the right ven-tricular branch of the right coronary artery anddraining into the right ventricle (Figure 4).None of the patients developed clinicalsymptoms or hemodynamic effects. Post-EMBechocardiograms showed no signs of right ven-tricular overload, and so a conservativeapproach was adopted. The last two caseshave not yet undergone repeat angiographiccontrol for comparative study.Several series have shown an incidence ofcoronary fistula in heart transplant patientsgreater than in the general population (5-8%Figura 3. Os dois últimoscasos ainda não realizaram um novo controloangiográfico para estudo evolutivo.Efectivamente várias séries têm demons-trado uma incidência de fístulas coronáriasem doentes transplantados cardíacos superiorà da população geral (5 a 8% versus 0.2%)(1,2).Este facto é atribuído ao elevado número defragmentos colhidos no decurso das múltiplasBEM a que os transplantados são submetidospara monitorização da ocorrência de rejeiçãoimunológica. A presença de arteríolas dilata-das nos fragmentos biopsados comprova issomesmo (3). A maioria das fístulas coronárias édetectada acidentalmente no decurso doprimeiro ano pós transplante, aquando darealização da primeira angiografia coronária.A sua origem é habitualmente a nível dadescendente anterior ou da coronária direita eo local de drenagem mais comum é a cavidadeventricular direita(4). Raramente causam sin-tomas, pois os shunts resultantes são habitual-mente pequenos. A sua evolução é benigna,havendo inclusivamente casos descritos deremissão espontânea (2). Como tal, na maioriados casos não é necessária qualquer inter-venção terapêutica. A incidência desta complicação tem vindoa diminuir ao longo dos últimos anos, prova-velmente consequência da experiência cres-cente dos operadores, da colheita de menornúmero de fragmentos em cada BEM e da res-trição das mesmas ao septo interventriculardireito (5).vs. 0.2%) (1, 2). The large number of fragmentscollected during multiple EMBs performed inheart transplant patients to diagnose immunerejection explains this difference. The pres-ence of dilated arterioles in biopsy supportsthis hypothesis (3). Most coronary fistulas aredetected accidentally in the year after trans-plantation, during the first coronaryangiogram. Their origin is usually in the ante-rior descending or right coronary artery andthe most common drainage site is the rightventricular cavity (4). Symptoms are rarebecause the resulting shunts are usuallysmall. The natural history of coronary fistulasis generally benign, and there are even casesreported of spontaneous closure(2).
