Iaizzo, PHDaD espite improvements in transcatheteraortic valve replacement (TAVR), post-TAVR percutaneous coronary interventionand periprocedural coronary occlusion represent ma-jor issues. To assess the interactions between TAVRprostheses, coronary ostia, and percutaneous coro-nary intervention devices, we tested TAVR-relatedcoronary techniques using Visible Heart methodolo-gies (1). This study complied with the guidelines bythe Institutional Animal Care and Use Committee(University of Minnesota). The selected TAVR systemwas Evolut self-expandable prosthesis (Medtronic,Minneapolis, Minnesota). Procedures were conductedunder fluoroscopic guidance by 2 expert interven-tional cardiologists, while endoscopic visualizationwas simultaneously recorded. Coronary cannulationand percutaneous coronary intervention were per-formed after single Evolut self-expandable prosthesisimplantation and after transcatheter aortic valve–in–transcatheter aortic valve. The collected endoscopicfootages highlighted the jailed coronary artery ostiaappearance and the optimal cannulation achieved inchallenging scenarios (Figures 1 and 2, Videos 1 and 2).Furthermore, 2 stent-based techniques for intrapro-cedural coronary occlusion prevention/managementwere tested.
