Efectividad del Protocolo Terapéutico Aplicado a Pacientes con Accidente Ofídico Atendidos en el Cuarto de Urgencias de un Hospital Público Ubicado en la Provincia de Veraguas. Enero-diciembre, 2021
A quantitative study was carried out with the aim of determining the level of effectiveness of the therapeutic protocol applied to patients treated with snakebite in the Emergency Room of the Luis Chicho Fábrega Hospital, in the period corresponding to January-December 2021. The study was descriptive, cross-sectional and retrospective, with a universe of 235 male and female cases, of which all patients who met the inclusion criteria were taken as a sample, made up of 194 cases. The Microsoft office Word 2017 program was used to write the final report, the SPSS statistical program and Excel 2013 for making graphs and tables. The information was obtained through the application of the Clinical management according to therapeutic standard instrument by the authors Solís-Tinoco with the direct review of the patients' clinical records. The compliance analysis of the protocol was carried out based on the four fundamental pillars of therapeutic management: area of ??bite location, indication of laboratory tests, administration of anti-venom serum and antimicrobial coverage. The results determined that the identification of the clinical picture according to signs and symptoms presented by the patient in relation to their level of toxicity, according to the applied protocol were: 23% cases of absent poisoning, 33% mild, 40% moderate and 4% severe. It was concluded that therapeutic compliance according to the Standard of care for poisoned patients in such cases was 77.3% in relation to the administration of anti-venom serum, the performance of laboratory tests and complementary examinations necessary for the evaluation of the damage of toxins by the venom to the different organs was 100%, the same percentage for antimicrobial coverage.Se realizó un estudio cuantitativo, con el objetivo de determinar el nivel de efectividad del protocolo terapéutico aplicado a pacientes atendidos con accidente ofídico en el Cuarto de Urgencias del Hospital Luis Chicho Fábrega durante el período correspondiente a enero-diciembre 2021. El estudio fue de tipo descriptivo, de corte transversal y retrospectivo, con un universo de 235 casos de sexo masculino y femenino, de los cuales se tomó como muestra a todos los pacientes que cumplieron con los criterios de inclusión, conformados por 194 casos. Se utilizó el programa Microsoft office Word 2017 para la redacción del informe final, el programa estadístico SPSS y Excel 2013 para la realización de gráficos y tablas. Se obtuvo la información mediante la aplicación del instrumento Manejo clínico según norma terapéutica de los autores Solís-Tinoco con la revisión directa de los expedientes clínicos de los pacientes. El análisis del cumplimiento del protocolo se realizó con base a los cuatro pilares fundamentales del manejo terapéutico: área de localización de la mordida, indicación de pruebas de laboratorios, administración del suero antiofídico y la cobertura antimicrobiana. Los resultados determinaron que la identificación del cuadro clínico de acuerdo con signos y síntomas presentados por el paciente en relación con su nivel de toxicidad, según el protocolo aplicado fueron: 23% casos de envenenamiento ausente, 33% leve, 40% moderado y 4% severos. Se concluyó que el cumplimiento terapéutico según la norma de atención a pacientes intoxicados en dichos casos fue de 77.3% en relación con la administración del suero antiofídico, la realización de pruebas de laboratorios y exámenes complementarios necesarios para la evaluación del daño de las toxinas por el veneno a los diferentes órganos fue del 100% igual porcentaje para la cobertura antimicrobiana
