La hernia de Amyand, una presentación rara de hernias inguinales, se caracteriza por la inclusión del apéndice vermiforme en el saco herniario. Esta condición, que representa aproximadamente el 1% de todas las hernias inguinales, suele diagnosticarse de manera intraoperatoria debido a su presentación inusual y a la dificultad para identificar preoperatoriamente. Se presenta el caso de un paciente masculino de 56 años con una hernia inguinal encarcelada que contenía colon, ciego, apéndice e íleon distal. Durante la cirugía, se realizó una apendicectomía y reparación con malla de la hernia. El postoperatorio fue satisfactorio, sin complicaciones. Este caso refuerza la seguridad del uso de malla en la reparación de hernias inguinales combinada con apendicectomía, incluso en ausencia de inflamación del apéndice, destacando la necesidad de un manejo individualizado. This condition, accounting for approximately 1% of all inguinal hernias, is typically diagnosed intraoperatively due to its unusual presentation and the difficulty of identifying it preoperatively. We present the case of a 56-year-old male patient with an incarcerated inguinal hernia containing colon, cecum, appendix, and distal ileum. During surgery, an appendectomy was performed along with mesh repair of the hernia. The postoperative course was satisfactory, with no complications. This case supports the safety of using mesh for inguinal hernia repair combined with appendectomy, even in the absence of appendiceal inflammation, and highlights the importance of individualized management
