Inzidenz der Pankreasfistel nach segmentaler Pankreastransplantation

Analyzing 30 nephrostomies of transplanted kidneys, we found a higher incidence of bleed- ing (17 %) and calicocutaneous fistula. There were no severe complications with loss of the graft or the recipient, even in immunosuppressed patients. Temporary percutaneous nephrostomy obviated the need for operation in 25 % of patients with obstructive disease. Long-term nephrostomy allows the exact evaluation of renal function after 1-2 months. Eine retrospektive Analyse von 30 Nephrostomien bei Transplantatnieren z igte eine vergleichsweise h6here Incidenz von Blutungen (17 %) und Kelchhautfisteln. Trotz Immunsuppres- sion trat keine Komplikation auf, die zum Verlust des Transplantates oder sogar des Empfiingers fiihrte. In 25 % konnte bei obstruktiver Erkrankung durch tempor~ire PCN eine Operation umgangen werden. Wie die Verl~iufe nach Langzeitfistelung zeigen, kann die Funktionswiederaufnahme erst nach 1-2 Monaten endgfiltig beurteilt werden. The occurrence of a pancreatic fistula is a major complication of pancreatic transplantation. It occurs, at least intermittently, despite occlusion of the pancreatic duct with prolamine. Compared to partially extra- and intraperitoneal placement, strict intraperitoneal placement showed no significant reduction in the incidence of fistula. The expected reabsorption ofexocrine secretions by the peritoneum therefore seems to be unsufficient. However, the incidence of graft failure after infected fistulas was lower, probably due to peritoneal defensive mechanisms. Der Erfolg einer Pankreastransplantation ist u.a. durch die Komplikation einer Pankreasfistel gef~ihrdet, die trotz Gangocclusion des Transplantates - zumindest voriibergehend - auftreten kann. Die Modifikation der Op.-Technik yon der teils extra-, teils intraperitonealen Plazie- 867 rung zur streng intraabdominellen Plazierung des Pankreastransplantates fiihrte jedoch zu keiner Reduktion der Incidenz von Pankreasfisteln. Die Resorptionsf'fihigkeit des Peritoneums ffir die vonder Drtise sezernierten Pankreasenzyme erscheint demnach nicht ausreichend. Die Abwehrfunktion des Peritoneums gegen bakterielle Infektionen - ausgehend yon der infizierten Pankreasfistel -, scheint jedoch zu einer geringeren Transplantatverlustrate zu fiihren. The viability of osteochondral grafts following cryopreservation at various temperatures ( -  20 °, - 70 °, - 196°), and by additional application of cryoprotectants, with duration of storage up to 3 months, was studied with the help of 3sS autoradiography. All cryopreserved grafts were proved to be nonvital, irrespective of the freezing technique used, by the failure to take up 3sS. Nonvitality of the grafts seems to be the basic reason for the osteoarthrosis of both graft and host tissue that is seen following midterm and long-term periods of observation i  animal experiments., Key words: Osteochondral grafts - Cryopreservation - Autoradiography (35S). Zusammenfassung. Mit Hilfe der Autoradiographie (S35) wurde die Vitalitiit osteochondraler T ans- plantate nach Konservierung in verschiedenen Temperaturbereichen ( -  20 °C, - 70 °C, - 196 °C) unter Anwendung von Gefrierschutzl6sungen und unterschiedlichen Lagerungszeiten (bis 3 Monate) fiberpriift. Ungeachtet der Gefriertechnik sind alle k~iltekonservierten Transplantate avital, was durch die fehlende Aufnahme von S 35 bewiesen wird. Dies scheint die haupts~ichliche Ursache der arthroti- schen Verfinderungen vo  Transplantat und Wirtsgewebe zu sein, vor allen Dingen wie sie bei mittelfri- stigen und Langzeitbeobachtungen im Experiment gefunden werden. L rsch  Klinik ffir Plastische Chirurgie und Anatomisches Institut der Medizinischen Universit~it zu Lfibeck, Ratzeburger Allee 160, D-2400 Liibeck Revascularisation of Isolated Autogeneous Bone Grafts by Microsurgical Implantation ofVascular Bundles Summary. Microsurgical implantation of saphenous vascular bundles into free autogeneous, isolated rib grafts in beagles resulted in revascularization f the bone after 2, 3, and 4 weeks. The newly formed vessels were demonstrated by injection of India ink, and in corrosion casts. Semithin sections revealed osteoblasts and osteoclasts in the vascular channels of the cortex, whereas in control rib grafts without implantation of blood vessels, only the matrix was observed. We saw no necrotic zones in the revascu- larized bones. The newly formed vascular networks consisted of arterial and venous vessels and their anastomoses.
