Increasing public health and primary care integration in Lane County, Oregon: a quality improvement project

Abstract

Primary care and public health have largely functioned independently of one another despite their intersecting goals and efforts to improve the health of shared populations. Following the Institute of Medicine's (IOM) 2012 "all to action" for integration of primary care and public health, the Lane County Public Health (LCPH) department in Lane County, Oregon sought to establish a more systematic process for increased collaboration and communication with clinical providers in their county by conducting a baseline needs assessment
