Pregnant women with diabetes: Health problems, APN interventions in 2 models of APN transitional care

Abstract

The purpose of this presentation is to examine differences in type and frequency of health problems and advanced practice nurse (APN) interventions between 2 models of APN provided prenatal care in pregnant women with diabetes. In one model, APN care was added to physician prenatal care (additive). In the second, half of physician prenatal care was substituted with APN prenatal care in women\u27s homes (substitution). For pregnant women with diabetes, maintaining diets and blood sugar levels is challenging. The literature lacks data on health problems from the women\u27s perspectives and if provider responses differ by model of care delivery
