Abstract ‘’Etoposide is a podophyllotoxin derivative with a wide range anti-cancer activity. However its use in clinical application is limited due to its cardiotoxicity side effects. Although several studies have been performed to uncover the cardiotoxicity mechanisms of anti-cancer drugs like Doxorubicin, Daunorubicin and Mitoxantrone, the Etoposide-associated cardiotoxicity remains unexplored. The differentiation of human pluripotent stem cells (hPSCs) into functional cardiomyocytes (hPSC-CMs) provides a powerful in vitro model system for analyzing the molecular mechanisms involved in drug-induced cardiotoxicity. The present study is focusing on uncovering the molecular mechanisms of Etoposide-associated cardiotoxicity using hiPSC-CMs. Moreover, the applicability of genomic and miRNA cardiotoxicity biomarkers applying an in vitro assay has been explored. Using hiPSC-CMs in combination with xCELLigence RTCA Cardio system an in vitro single-dose model for testing Etoposide-induced cardiotoxicity has been successfully developed. Additionally, applying the quantitative RT-PCR analysis 58 deregulated genes and 5 deregulated miRNAs have been identified in hiPSC-CMs treated with Etoposide for 48 h. Bioinformatics analysis of these deregulated genes and miRNAs showed their involvement in vital cardiac cellular processes like cytoskeletal organization, muscle contraction, Ca2+ ion homeostasis and induction of apoptosis.  Immunostaining and Transmission Electron Microscopy based analysis confirmed the cytoskeletal and mitochondrial damage in CMs treated with Etoposide. Etoposide exposure lead to increased ROS production, altered Ca2+ ion homeostasis and activation of p53-mediated ferroptosis in CMs. Etoposide-induced mitochondrial damage was found to be crucial for the induction of cardiotoxicity and the small molecule inhibitor of p53 signaling, Pifithrin-α and ferroptosis inhibitor, Liproxstatin-1 were found to improve the CMs function after Etoposide treatment. In conclusion, this study shows that single high-dose of Etoposide induce cardiotoxicity in hiPSC-CMs primarily via damaging the mitochondria and deregulating genes involved in vital cellular processes. In addition the genomic markers identified in this study can also be applied for the cardiac safety assessments of novel chemical entities in vitro.’’ (Nemade et al. 2018)              Zusammenfassung Etoposide ist ein Podophyllotoxin-Derivat mit einer breiten Antitumor-Aktivität.  Nichtdestotrotz ist die klinische Anwendung von Etoposide aufgrund kardiotoxischer Effekte eingeschränkt. Trotz der Durchführung mehrerer Studien zur Aufklärung der kardiotoxischen Mechanismen von Antitumor-Medikamenten wie z.B. Doxorubicin, Daunorubicin und Mitoxantrone, sind die Mechanismen der Etoposide-assoziierten  Kardiotoxizität weitgehend unerforscht. Die Differenzierung von humanen induzierten pluripotenten Stammzellen (hiPSCs) zu funktionellen Kardiomyozyten (hiPSC-CMs) ist ein potentes in vitro Modellsystem für die Analyse der molekularen Mechanismen medikamenten-induzierter Kardiotoxizität. Die vorliegende Studie unter Einsatz von hiPSC-CMs ist auf die Aufklärung von molekularen Mechanismen der Etoposide-induzierten Kardiotoxizität fokussiert. Darüber hinaus wurden genomische und miRNA-Marker für ein in vitro-Kardiotoxizitäts-Testsystem identifiziert. Durch die Anwendung von hiPSC-CMs in Kombination mit dem xCELLigence RTCA Cardio-System wurde ein in vitro Einzeldosis-Testsystem für die Etoposide-induzierte Kardiotoxizität etabliert. Darüber hinaus wurden mittels  quantitativer „real-time polymerase chain reaction“ (qRT-PCR) 58 deregulierte Gene und 5 deregulierte miRNAs nach Behandlung der hiPSC-CMs mit Etoposide für 48 Stunden nachgewiesen. Die bioinformatische Analyse der deregulierten Gene und miRNAs zeigte, dass diese genomische Marker in wichtigen zellulären Prozessen  wie z.B. bei der Organisation des Zytoskeletts,  der Muskelkontraktion, der Ca2+ Homöostase und bei apoptotischen Prozessen involviert sind. Immunhistochemische und transmissionselektronenmikroskopische Untersuchungen bestätigten die Zytoskelett- und Mitochondrialen-Schäden, die durch Behandlung von Kardiomyozyten mit Etoposide induziert wurden. Behandlung der Kardiomyozyten mit Etoposid führte zu einer erhöhten Produktion von sogenannten “reactive oxygen species” (ROS), zu Veränderungen der Ca2+-Homöostase und schließlich zur Aktivierung p53-vermittelter Ferroptose in  hiPSC-CMs. Die Etoposide-induzierten mitochondrialen Schäden sind sehr kritisch für die Induktion der Kardiotoxizität. Die Behandlung der Zellen mit dem Kleinmolekül-Inhibitor des p53-Signalweges Pifithrin-α und mit dem Ferroptoseinhibitor Liproxstatin-1 verbesserte die Funktion der hiPSC-CMs  nach Behandlung der Zellen mit Etoposide. Zusammenfassend zeigt die vorliegende Arbeit, dass Etoposide-Hochdosis-induzierte Kardiotoxizität in hiPSC-CMs primär durch eine Schädigung der Mitochondrien einhergehend mit einer Deregulierung von Genen und miRNAs, die für die Funktion von Kardiomyozyten essenziell sind, vermittelt wird. Darüber hinaus können genomische Toxizitätsmarker als in vitro Kardiotoxizitätsmarker für Sicherheitsstudien eingesetzt werden.
