Abstract Pluripotent stem cell (PSC)-derived cardiomyocytes (CMs) represent a promising cell type for functional regeneration of infarcted heart. The long-term success of this therapeutic approach depends on the acceptance of the donor tissue by the host immune system. Therefore, the main objective of this study was to elucidate the immunological properties of murine embryonic stem cell (ESC)-derived CMs in vivo and in vitro. After we validated that the kidney capsule supports long-term ESC-CM survival and that it is immunocompetent, we used this transplantation site to transplant ESC-CM clusters to minimizing cell loss. Thereby immunocompetent allogeneic mice mismatched for major histocompatibility complex (MHC-) or minor histocompatibility (miH)-antigens were used to investigate potential differences in the allogeneic response based on the type of antigen. For that purpose the transgenic αPIG44 ESC line expressing an antibiotic resistance gene and a green fluorescent protein (GFP) under the control of a cardiac gene promoter, allowing purification and tracking of CMs, was used. In order to monitor the specific cytotoxic T cell (CTL) response to a defined miH-antigen this cell line was additionally modified to constitutively express the ovalbumin protein (OVA). We semi-quantified the transplanted ESC-CMs by the detection of their GFP signal and determined the number of infiltrating cluster of differentiation (CD) 45+ lymphocytes, CD3+ T cells and F4/80+ macrophages. We found that ESC-CMs survived for at least 8 weeks in immunodeficient and MHC-mismatched BALB/c mice. However, ESC-CMs transplanted into the MHC-mismatched mouse strain FVB/N were fully rejected within 8 weeks accompanied by a significant leukocyte infiltration. The miH-mismatched OVA-expressing ESC-CMs survived for 8 weeks in naïve mice. Surprisingly, OVA-immunization did not reduce the survival of transplanted ESC-OVA-CMs, although it induced OVA-specific CTLs which caused reduction of OVA-expressing melanoma cells. In vitro assays revealed that ESC-CMs were not generally able to escape CTL killing, but killing was hindered compared to control cells. These findings deliver first hints to explain why miH-antigen disparity may not be sufficient to cause rejection of ESC-CMs even in the presence of active antigen-specific CTLs and they highlight that the immune response to MHC-mismatched cells seems to be strongly depended on the genetic background of the allogeneic inbred mouse recipients. Zusammenfassung Für die funktionelle Regeneration des infarktgeschädigten Herzens, gilt der Einsatz von aus pluripotenten Stammzellen differenzierten Kardiomyozyten (CMs) als vielversprechend. Der Langzeiterfolg dieses Therapieansatzes ist abhängig von der Akzeptanz des Spendergewebes durch das Immunsystem des Empfän-gers. Deshalb war das Ziel dieser Studie die immunologischen Eigenschaften von aus murinen embryonalen Stammzellen (ESCs) differenzierten CMs in vivo und in vitro zu bestimmen. Nachdem feststand, dass die Nierenkapsel immunkompetent und das Langzeitüberleben von ESC-CMs gewährleistete, nutzten wir sie als Transplantationsort. Bei den Transplantationsexperimenten wurden immunkompe-tente Mäuse mit abweichendem Haupthistokompatibilitätskomplex (MHC) oder Minor Histokompatibilitätsantigenen (miH) verwendet, um etwaige Unterschiede der allogenen Immunantwort zwischen diesen beiden Antigentypen zu erkennen. Zur Aufreinigung und Verfolgung der ESC-CMs wurde die transgene ESC-Linie αPIG44 verwendet, welche eine Antibiotikaresistenz und ein grün fluoreszierendes Protein (GFP) unter Kontrolle eines kardialen Promoters exprimiert. Um die Ant-wort einer spezifischen zytotoxischen T-Zellanwort (CTL) auf ein definiertes miH-Antigen feststellen zu können, wurde die ESC-Linie zusätzlich modifiziert das Ovalbuminprotein (OVA) konstitutiv zu exprimieren (ESC-OVA). Wir semiquanti-fizierten die transplantierten ESC-CMs und ermittelten die Anzahl infiltrierender Lymphozyten, T-Zellen und Makrophagen. Wir fanden heraus, dass ESC-CMs für mindestens 8 Wochen in immundefizienten und MHC-abweichenden BALB/c-Mäusen überleben, wohingegen sie in MHC-abweichenden FVB/N-Mäusen, begleitet von einer signifikanten Leukozyteninfiltration, vollständig abgestoßen wurden. Die in miH-abweichenden ESC-OVA-CMs überlebten 8 Wochen in naiven Mäusen des gleichen Haplotyps. Ihr Überleben reduzierte sich durch eine Immunisierung mit OVA nicht, obwohl OVA-spezifische CTLs induziert worden waren, welche Wachstum von OVA-exprimierenden Melanomas verminderten. In vitro Assays ergaben, dass ESC-CMs der Lyse durch CTLs nicht grundsätzlich entkamen, aber diese im Vergleich zu den Kontrollzellen vermindert war. Diese Ergebnisse liefern erste Hinweise dafür, warum miH-abweichende ESC-CMs nicht abgestoßen werden, selbst wenn aktive, antigen-spezifische CTLs anwesend sind und dass die Immunantwort auf MHC-abweichende Zellen stark vom genetischen Hintergrund des Mausstammes abhängt.
