In Brazil, the National Land Transportation Agency (ANTT in Portuguese) Resolution 16/02 defines semiurban road passenger transport as journeys with urban transport characteristics that are as long as 75km (or less) and that cross the State, Federal District or Territory border. Even though this type of service is regulated, Decree 2.521/98 does not present a specific approach for this service, which may jeopardize its regulatory policy and service planning. Besides, the lack of a specific approach allows for user expectations to remain unfulfilled. Thus, regarding land passenger transport, the main problem we spotted was the lack of knowledge on semiurban road passenger transport. To consider the perceptions of users, planners and operators involved in semiurban transport is essential to understand how it works and, subsequently, to characterize it. From this point of view, Content Analysis comes to light as a specific technique that aims to describe content objectively, systematically, qualitatively and quantitatively in order to build a System of Synthetic Categories for each actor in semiurban transport. Trabalho final do 6º ano médico com vista  à atribuição do Grau de mestre no âmbito do ciclo de estudos de mestrado integrado em medicina da Faculdade de Medicina da Universidade de Coimbra.A hérnia de disco intervertebral com compressão nervosa que não cede ao tratamento médico, tradicionalmente, é indicação para tratamento cirúrgico. No entanto, técnicas minimamente invasivas e os procedimentos percutâneos têm sido usados e desenvolvidos nas últimas duas décadas, tanto como procedimentos alternativos, bem como possibilidades terapêuticas que antecedem o tratamento cirúrgico convencional, podendo adiar o mesmo por tempo indeterminado. De entre estas opções técnicas, surge a Nucleotomia por Laser (Percutaneous Laser Disc Decompression).
Objectivos: É efectuada uma compilação da história e estudos realizados no âmbito da Nucleotomia por Laser. Descrevem-se os procedimentos usados para a realização desta técnica e seus resultados recorrendo não só a estudos publicados como também à experiência adquirida no serviço de Ortopedia do Centro Hospitalar do Nordeste – Unidade de Macedo de Cavaleiros.
Desenvolvimento: O princípio do tratamento da Nucleotomia por Laser baseia-se no conceito do disco intervertebral ser um sistema hidráulico fechado. Uma diminuição na pressão intradiscal resultará na redução do material protusado em direcção ao centro discal e consequentemente, à minimização da compressão exercida sobre a raiz nervosa e ao alívio sintomático.
Em todo o mundo, o laser Nd:YAG é o mais usado. Tem os maiores estudos de follow-up e o maior historial de uso seguro, com o menor número de complicações.
O doente habilitado a ser seleccionado para ser submetido a esta técnica é todo aquele em que não se obteve qualquer alívio da dor lombar irradiada para um ou para os dois membros inferiores decorridas 6 semanas de tratamento conservador.
A classificação mais usada, que permitirá aferir a taxa de sucesso, é a efectuada sob a forma de resultados funcionais - os critérios de MacNab.

Neste trabalho faz-se referência à técnica e à série de 225 Nucleotomias por Laser realizadas em Macedo de Cavaleiros de Janeiro de 2006 a Julho de 2009 e discutem-se os resultados obtidos.
Conclusões: A Nucleotomia por Laser é um procedimento minimamente invasivo com duração de intervenção reduzida, usado em ambiente de ambulatório, alta eficácia e baixos índices de complicações. É uma modalidade terapêutica para casos seleccionados de protusão discal que não cede a tratamento conservador. Esta técnica, embora carecendo ainda de comprovação científica bem estruturada, surge como alternativa a procedimentos cirúrgicos tradicionais.Traditionally, intervertebral disc herniations with radicular compression that are not resolved with medical treatment have surgical indication. However there are minimally invasive techniques and percutaneous procedures that have been developed and used in the last two decades as an alternative to major surgery. They are also used as therapeutic procedures, to be considered as a preliminary option, preceding and postponing surgical intervention. An example of such a technique is Percutaneous Laser Disc Decompression.
Objectives: To do a literature review on Percutaneous Laser Disc Decompression studies. To describe the reported methods of this technique and there results, resorting not only to published studies, but also to the experience acquired in the Serviço de Ortopedia do Centro Hospitalar do Nordeste – Unidade de Macedo de Cavaleiros, Portugal.
Furthermore: The principle of treatment of Percutaneous Laser Disc Decompression is based on the concept of the intervertebral disc being a closed hydraulic system. A decrease in

the internal disc pressure will induce the reduction of the protruded formation towards the centre of the disc, consequently minimizing the initial compression exerted on the nervous root and thus producing symptomatic relief.
Around the world Nd:YAG laser in the most used. It has the biggest follow-up studies and the largest history of safe use, with the least number of complications.
The ideal patient to be selected for the application of this technique is the one who hasn’t experienced any kind of relief of the lumbar pain radiating to one or to both inferior limbs after 6 weeks of conservative treatment.
The most common classification used, which allows to check the success rates, is carried out in the form of functional results - the MacNab criteria.
In this paper the results obtained in a series of 225 Percutaneous Laser Disc Decompression completed in the Unidade de Macedo de Cavaleiros, between January, 2006 and July, 2009 are discussed.
Conclusions: Percutaneous Laser Disc Decompression is a minimally invasive procedure with: a short period intervention time needed, capable of use in an ambulatory environment, with high efficiency and with low rates of complications. It’s specific for cases of disc protrusion that are not resolved through conservative measures. Although this technique, still needs more structured scientific proof, it is emerging as an alternative for traditional surgical proceedings
