本研究は日本人が経験している医師との関わりの中から、信頼と不信の要因を明らかにすることを目的として行われた。25名の成人男女に対して半構造化面接を実施し、グラウンデッドセオリーアプローチの手法を用いて分析したところ、①医師の医学的能力に関する要因、②医師の態度・言動に関する要因、③医師-患者の感情、コミュニケーションに関する要因の3つのカテゴリーが見出された。それらはさらに①―1.医師についての評判・伝聞、①―2.医師の個人的・社会的特牲、①―3.適切な処置と治療の結末、②―1.医師の診療態度・接遇、②―2.十分な説明と納得、②―3.患者の利益の優先、②―4.背景要因となる限界性、③―1.医師の配慮・共感、③―2.医師のコミュニケーション能力と疎通性、③―3.患者の感情、の10のサブカテゴリーに分類された。医療に対する信頼の場合、能力と意図を切り離すことは難しく、感情的側面が重要な要因であることが明らかになった。The development and maintenance of trust in the physician-patient relationship is essential for the practice of high quality health care. However, distrust of medicine has been more serious in Japan recently. The purpose of this study is to explore the factors of patient trust and distrust in physicians among Japanese. We conducted 25 semi-structured interviews with Japanese citizens based on grounded theory approach. Categories regarding patient trust included (1)physician's competency, (2)appropriate responses to patients, and (3)emotional aspects of the physician-patient relationship. Ten sub-categories were derived from these categories, which included physician's reputation, personal and social characteristics of the physician, outcomes of the treatment, physician's appropriate manner, full disclosure of medical information and informed consent, respect for patients' interest, limit of providing enough care, sympathy and compassion, physician's communication skill and communicability, patients' emotions. The result of our study shows that some parts of the patient trust we figured out were similar to the factors found in the previous studies done in western countries. However, some parts of the sub-categories we found out seems unique to our culture. More detailed analysis on characteristics of Japanese interpersonal relationship will be needed
