Opinnäytetyön aiheena oli toiminnan muutos Kuopion psykiatrian keskuksen (KPK) päivystyksessä - kysely hoitajille. Opinnäytetyön tarkoituksena oli tutkia ja kuvata päivystystyötä tekevien KPK:n sairaanhoitajien näkemyksiä psykiatrisen päivystystoiminnan muutoksesta KPK:ssa. Tavoitteena oli saada tietoa nykyisestä psykiatrian päivystyksen toimivuudesta ja mahdollisista kehittämistarpeista. Opinnäytetyön yhteistyökumppanina oli Kuopion psykiatrian keskus, joka tuottaa ja järjestää psykiatrisen erikoissairaanhoidon avohoidon tutkimus-, kuntoutus- ja hoitopalveluja Pohjois-Savon sairaanhoitopiiriin kuuluville työikäisille asiakkaille.
Kuopiossa mielenterveydellisissä ongelmissa apua haetaan ensisijaisesti omalääkäriltä terveyskeskuksen kautta, josta on mahdollista saada lähete jatkohoitoon Kuopion psykiatrian keskukseen. Akuuteissa tilanteissa psykiatrinen potilas hakeutuu tai ohjataan muista palveluista Puijon sairaalan yhteispäivystyksen poliklinikalle. Yhteispäivystyk-sen tiloissa toimii psykiatrisen sairaanhoitajan päivystysvastaanotto, jossa potilaiden avun tarvetta arvioidaan moniammatillisena tiiminä.
Keskeisin Kuopion psykiatrian keskuksessa tapahtunut muutos liittyy päivystystoiminnan järjestämisen hoitokäytännön muutokseen marraskuussa 2013: KPK:n työryhmät alkoivat hoitaa omat akutisoituneet eli päivystyksellistä apua tarvitsevat potilaat omissa, sovituissa työryhmissään. Ennen muutosta päivystyksellistä apua tarvitsevat potilaat oli hoidettu siten, että he olivat ohjautuneet päivystystyöparille, joihin hänellä ei välttämättä ole aikaisemmin ollut hoitokontaktia. KPK:n päivystävän hoitajan rooli on tänä päivänä toimia linkkinä potilaan ja eri toimijoiden välillä.
Tutkimus tehtiin toteuttamalla internet-pohjainen kysely Webropol-ohjelmalla. Kohderyhmänä olivat kaikki KPK:ssa päivystystyötä tekevät sairaanhoitajat. Kyselyn sisältö laadittiin yhteistyössä KPK:n kanssa ja se vastasi eri osiossa sekä määrällistä että laadullista tutkimusta. Laadullinen aineisto analysoitiin aineistolähtöisellä sisällönanalyysilla.
Tulosten perusteella päivystystoiminnan muutos herätti monenlaisia mielipiteitä. Pääosin päivystystoiminnassa tapahtuneeseen muutokseen suhtauduttiin myönteisesti ja toiveikkaasti. Vastauksien laajuuden perusteella voidaan summata aineiston olevan rikasta, koska kyselyyn vastaajat olivat selkeästi pohtineet monipuolisesti omia näkemyksiään päivystyksen toiminnasta.
Tuloksia voidaan hyödyntää päivystystoiminnan kehittämisessä. Valmiin työn kautta KPK saa tutkimuksellista tietoa siitä, kuinka muutosta toteuttavat hoitajat suhtautuvat muuttuneeseen tilanteeseen työssään.The subject of the thesis was the organizational change of the emergency duties of the Kuopio Psychiatry Centre (KPC). The purpose of this study was to explore and describe the views and opinions that KPC's psychiatric nurses have about the organizational changes. The aim was to learn about the quality of the current psychiatric emergen-cy services and any developmental needs about the field. The partner was The Kuopio Psychiatric Centre, which produces and organizes specialized psychiatric outpatient care and outpatient rehabilitation services for working age customers in the Kuopio University Hospital district.
In Kuopio the citizens can find help for mental problems primarily by contacting a primary health care physician. A general practitioner can make a referral to a psychiatrist of KPC. In acute situations, patients from the Puijo region are directed to or can directly contact the Emergency Department of Puijo Hospital, i.e. the University Hospital of Kuopio. In the Emergency Department there is a practice of psychiatric nurse and a patient´s need for help is evaluated by the multi-professional team.
The most important changes in KPC’s emergency duties was the change in the action of psychiatric care in November 2013: KPC’s working professional groups agreed to take care and manage their own patient’s acute situations. Before the organizational change the patients in need for acute help had to contact or were directed to contact an on-call psychiatric nurses, which may not have worked before with that particular patient. Nowadays, in the KPC, the role of the nurse on duty is to work as a link between the patient and the different parties of the psychiatric care.
The research was conducted by implementing an internet-based questionnaire using Webropol Programme. The target groups were all nurses of the KPC doing psychiatric on-call work. The survey content was set in cooperation with the KPC and it corresponded to the different sections of both quantitative and qualitative research. The qualitative data was analyzed by the inductive context analysis.
Based on the results of the thesis, the organizational change of the psychiatric emergency care aroused a wide range of opinions. Mainly the opinions of the psychiatric nurses were positive and hopeful. The survey respondents were especially considered and focused with the open questions of the survey, and the responses to the open questions were carefully argumented, assuring rich research material.
The results can be used in the development of the emergency duties and in the organizational changes of other working communities. With the finished thesis work the KPC receives research-oriented information about the ex-periences, attitudes and opinions of the health care professional workers who put in the practice the developmental changes of the working community which they belong to
