Tukimateriaalin tuottaminen iäkkäille insuliinihoitoista tyypin 2 diabetesta sairastaville venäjänkielisille maahanmuuttajille ja heitä ohjaavalle hoitohenkilökunnalle
Opinnäytetyömme tarkoituksena oli tuottaa tukimateriaalia iäkkäille tyypin 2 diabetesta sairastaville venäjänkielisille maahanmuuttajille. Opinnäytetyömme liittyy LOG-Sote-hankkeeseen
(Lokaali- ja globaali kehittäminen sosiaali- ja terveysalalla). Hankkeen tarkoituksena on kartoittaa maahanmuuttajataustaisen väestön terveystarpeita ja ongelmia terveyden edistämiseksi ja terveyspalvelujen kehittämiseksi. Opinnäytetyömme tulee käyttöön
hankkeen yhteistyökumppaneille.
Venäjänkielinen tukimateriaali (liite 3) toimii itsehoidon tukena kotioloissa ja tarvittaessa
päivittäisenä muistutuksena ikääntyneille potilaille. Hoitohenkilökunnan antaman ohjauksen tueksi sama tukimateriaali on myös suomenkielisenä (liite 2), jolloin se yhdessä venäjänkielisen
tukimateriaalin kanssa helpottaa molemminpuolista ymmärrystä ja vuorovaikutusta ohjaustilanteessa. Tukimateriaalin käytön edellytyksenä on, että ohjaajalla ja potilaalla on kohtalainen kielellinen ymmärrys. Tukimateriaalin ei ole tarkoitus korvata suullista ohjausta, vaan tarjota uuden työkalun sen tueksi. Tukimateriaalissa käydään läpi verensokerin mittaaminen, insuliinin pistostekniikka sekä matalan ja korkean verensokerin oireet. Tiedon sisäistämistä on tuettu havainnollistavien kuvien ja selkokielisten ohjeiden avulla.
Teoriaosuudessa perehdyimme venäjänkielisiin maahanmuuttajiin, ikääntyneen diabeetikon
erityispiirteisiin sekä hyvään potilasohjaukseen. Lisäksi käsittelemme laadukkaan kirjallisen
potilasohjeen tuottamista. Opinnäytetyömme näyttöön perustuvan teoriapohjan keräsimme
sekä suomenkielisistä että kansainvälisistä tietokannoista. Käytimme myös uusinta
aiheeseen liittyvää kirjallisuutta. Tukimateriaali on tuotettu pdf-muodossa ja tallennettu cd-rom–levykkeelle, jotta se olisi helposti välitettävissä ja käytettävissä.
Tukimateriaalin sisältö käydään yhdessä läpi ohjaustilanteessa, jonka jälkeen potilas saa
yksilöllisillä tiedoilla täydennetyn tukimateriaalin kotiin. Kotona potilas käyttää tukimateriaalia muistin tukena jokapäiväisessä itsehoidossa. Tukimateriaalin havainnollistavat kuvat voidaan laittaa näkyvälle paikalle helpottamaan muistamista.
Lisääntynyt tieto sairaudesta ja sen hoidosta sekä saatu tuki lisäävät potilaiden hoitoon
sitoutumista ja voimaannuttavat heidät sairauden hoitoon. Tukimateriaalin avulla iäkkäiden
tyypin 2 diabetesta sairastavien venäjänkielisten maahanmuuttajien itsehoito ja sairauden
hoitotasapaino paranevat. Tukimateriaali parantaa hoitohenkilökunnan ohjausvalmiuksia, jolloin venäjänkielisten potilaiden kokemukset sairauden hoidosta ja saadusta ohjauksesta
paranevat ja tyytyväisyys kasvaa.Our final project was executed in order to provide supportive material for the elderly with
type 2 diabetes mellitus among the Russian-speaking immigrants. Our final project relates
to the LOG SOTE project (Local and global development of social and health care). LOG SOTE project aims to identify the immigrant population’s health needs, problems and health remediation. Our final project will be available for project partners in the social care sector.
Language support material in Russian (appendix 3) was designed to aid self-care at home
and will also work as a reminder for elderly patients. Along with the Nursing staff's guidance,
supportive material written in Finnish (appendix 2) and Russian, will improve mutual
understanding and interaction between the patient and nursing personnel. A mutual language
between the patient and the nursing staff is a condition for the use. The supportive
material is not intended to replace the verbal guidance but to offer a new tool to support
the guidance. The supportive material consists of measurement of blood glucose, insulin
injection techniques, and high and low blood sugar symptoms. In order to provide easily absorbed self-care information and guidance, illustrative pictures and simple instructions
are produced. In the theoretical part, we familiarized ourselves with the Russian-speaking
immigrants, the elderly diabetic characteristics and good patient treatment. In our final
project we also explored the requirements and expectations of the high-quality patient
instructions. The theoretical and evidence-based data of our final project was gathered from Finnish and international databases. We also used the latest literature related to the topic. Supportive material was printed out in an electronic form (PFD) and stored on CD. The material is easily distributable and available.
The patient and the nurse will go through the content of the support material together after which the patient gets home the material supplemented with individual information. At home, the patient can use the support material as an everyday reminder of self-care.
The illustrative pictures can be put up on a visible place to make it easier to remember. Increased knowledge of the illness, knowledge on diabetes self-management and enhanced customer service will increase the patient self-care involvement. Supportive material for older people with type 2 diabetes, self-care -management and medical treatment are paramount to the treatment. The provided material will improve the nursing staff’s
instruction and guidance skills, whereas the Russian-speaking patients' experiences from
the disease treatment and general interactions between the patient and counseling will be
improved and the patient satisfaction grows
