Tämän opinnäytetyön tarkoituksena oli kehittää lääkehoidon perehdytystä Lohjan sairaalan kirurgisella vuodeosastolla ja tuottaa lääkehoidon perehdytystä tukeva muistilista. Muistilistan avulla haluttiin yhtenäistää lääkehoidon perehdytyksen sisältöä, selkeyttää työnjakoa sekä luoda suunnitelmallinen aikataulu perehdytyksen etenemiselle. Tavoitteena oli myös tuoda kirurgisen vuodeosaston lääkehoitosuunnitelma näkyväksi osaksi lääkehoidon perehdytystä.
Opinnäytetyö oli osa Laurea-ammattikorkeakoulun Lohjan toimipisteen, Lohjan sairaalan kirurgisen vuodeosaston, Hangon kotihoidon ja tanskalaisen Vejlen sairaalan yhteistä turvallisen lääkehoidon kehittämishanketta. Yhteistyö Lohjan sairaalan kirurgisen vuodeosaston kanssa aloitettiin keväällä 2009 ja työryhmään kuului kaksi Laurea-ammattikorkeakoulun sairaanhoidon opiskelijaa sekä neljä työelämän edustajaa.
Opinnäytetyön tuotoksena syntynyt muistilista perustuu Sosiaali- ja terveysministeriön vuonna 2005 julkaisemaan Turvallinen lääkehoito -oppaaseen sekä Lohjan sairaalan kirurgisen vuodeosaston lääkehoitosuunnitelmaan. Opinnäytetyön tietoperustan muita aiheita olivat potilasturvallisuus ja lääkehoidon perehdyttäminen. Opinnäytetyöhön liittynyt tiedonhakumatka Tanskaan toteutettiin tammikuussa 2010. Matkan tarkoituksena oli tutustua Vejlen sairaalan osaston A150 perehdytysohjelmaan ja tuoda sieltä hyviä käytänteitä omaan opinnäytetyöhön. Idea lääkehoidon perehdytystä tukevan muistilistan lähettämisestä uudelle työntekijälle ennakkoon saatiin tältä Tanskan matkalta.
Kehittämistoiminnan menetelmänä käytettiin juurruttamista, joka mahdollistaa näkyvän ja hiljaisen tiedon käsittelyn sekä uuden tiedon luomisen. Opinnäytetyön ensimmäisessä vaiheessa Lohjan sairaalan kirurgisen vuodeosaston lääkehoidon nykytilaa kartoitettiin teemahaastattelun avulla. Teemahaastattelun pohjalta laadittiin lääkehoidon eri osa-alueita kuvaavia käsitekarttoja, joista työyhteisö valitsi lopulliseksi kehittämiskohteeksi lääkehoidon perehdytyksen. Opinnäytetyön toisessa vaiheessa työryhmä kehitti yhdessä työyhteisön kanssa lääkehoidon perehdytystä tukevan muistilistan, sekä ehdotuksen lääkehoidon perehdytyksen työnjaosta ja aikataulusta perehdytyksen etenemiselle.
Työryhmän toimintaa ja opinnäytetyön tuotosta arvioitiin erillisen arviointikaavakkeen avulla. Vastausten perusteella työryhmän kehittämän muistilistan käyttöönottoon uskottiin erittäin paljon tai paljon. Työryhmän ehdottaman työnjaon ja aikataulun arvioitiin selkiyttävän osastonhoitajan, perehdyttäjän ja perehdytettävän rooleja sekä lääkehoidon perehdytyksen suunnitelmallista toteuttamista.
Lääkehoidon perehdyttäminen on keskeinen osa uuden sairaanhoitajan tai lähi- ja perushoitajan perehdytysohjelmaa. Tämän opinnäytetyön tuotosta voidaan jatkossa hyödyntää myös perehdytysohjelman muiden osa-alueiden perehdytyksen ja niitä tukevien perehdytysmateriaalien kehittämisessä. Myös työryhmän ehdottama perehdytyksen aikataulu ja työnjako on sovellettavissa koko osaston perehdytysohjelmaan.Development of orientation process of pharmacotherapy in Lohja hospital’s surgical bed ward
The purpose of this Thesis was to develop the orientation process of pharmacotherapy in Lohja hospital’s surgical bed ward and to produce a checklist that supports the orientation process. The goal of the checklist was to unify the content, clarify the division of responsibilities and to create a systematic schedule for the orientation process of pharmacotherapy.
This Thesis was part of the Safe pharmacotherapy project with Laurea University of Applied Sciences, Lohja hospital’s surgical bed ward, homecare unit in Hanko and Vejle hospital in Denmark. Collaboration with Lohja hospital’s surgical bed ward was started in spring 2009. Workgroup consisted two nursing students from Laurea University of Applied Sciences and four working life representatives.
The output of this Thesis is based on pharmacotherapy plan of Lohja hospital’s surgical bed ward and national guidelines for safe Pharmacotherapy by the Finnish Ministry of Social Affairs and Health. Other areas of the theoretical basis were patient safety and orientation process of pharmacotherapy. This thesis project included a research visit to Denmark. The goal of the visit was to benchmark good practices from the orientation program of Vejle hospital’s ward A150. The idea of sending the checklist to new staff member in advanced was picked from this visit.
Dissemination was used as method for development process in this Thesis. Dissemination enables to produce some new knowledge by processing tacit, practical and examined knowledge. At the first stage of this Thesis project working life representatives of the workgroup were interviewed for the present stage description of pharmacotherapy. Visual herringbone pattern that represents the present stage description was introduced to work community and they chose that the orientation process of pharmacotherapy should be the area of development. At the other stage of this theses project workgroup produced a checklist that supports the orientation process of pharmacotherapy. Suggestion of division of responsibilities and systematic schedule for orientation process was also made.
Actions of the workgroup and the product of this thesis were evaluated with an assessment form. According to answers, the change and the practical benefits of this thesis were considered significant or very significant Workgroup’s suggestion of division of responsibilities and systematic schedule for orientation process were considered to clarify the roles of the head nurse, mentors and orientees.
Pharmacotherapy is very important area of the orientation program for new nurses and practical nurses. The product of this thesis could also be used in development of the other areas of the orientation program. Workgroup’s suggestion of division of responsibilities and systematic schedule for orientation is suitable for entire orientation program
