Dismantling a butterfly gastroplasty: revision surgery of an uncommonbariatric procedureBackground: Gastric Bypass is the gold standard operation for bariatricsurgery. It is a good option for failed previous procedures and for salvage surgeryfor complications. The “Butterfly” gastroplasty, an uncommon bariatric proceduredescribed in Egypt, consists of a micro funnel shaped pouch constructedlimited to cardia using two endo-cutter cartridges. This is the first reported casefor revision surgery for this type of operation.Methods: We report the clinical case of a 59-year-old gentleman who underwenta laparoscopic Butterfly gastroplasty in Egypt. Patient seeked furthersurgery for the management of his weight regain and ongoing problems of nauseaand vomiting postprandially.Results: The patient underwent revisional surgery. This video shows thetechnical aspects of excision of the Butterfly deformity of the stomach and thenthe creation of a new gastric pouch, completing the gastric bypass afterwards.The patient tolerated the procedure well and was discharged home on the secondpostoperative date without any complications. He did not have any particularmorbidity during the first month after the operation.Conclusion: The Roux-en-Y gastric bypas is a good option for revisionalsurgery for uncommon operations such as the butterfly gastroplasty. It is afeasible option and usually extensive preoperative work out is necessary in thesepatients to identify the anatomy as these procedures are not standardized.
