  Introduction to IT Architectures and Implementations in Healthcare Environments Minitrack  Radmila Juric USN, Norway rju@usn.no  Karen Stendal USN Norway Karen.Stendal@usv.no  Sang Suh Texas A&M University- Commerce, US Sang.Suh@tamuc.eduThe minitrack on IT Architectures and Implementation in Healthcare Environments has attracted a variety of papers, but we selected only three. They are very different in terms of problems they address in developing healthcare software applications and technologies used in their implementations.  The paper entitled “Putting Interoperability on Health Information-systems’ Implementation Agenda” comes from Sweden and addresses a more than three decade old problem of heterogeneities in healthcare software solutions, which has been aggravated by the abundance of technologies we use in modern software applications, and hyper production of data we face on a daily basis. Heterogeneity is a normal characteristic of modern computing and problems of interoperability created by heterogeneity have to be addressed differently, from one specific situation to another. In this paper, the authors look at the health information systems (HIS) data exchange challenges, and propose a sense making perspective for analysing how HIS developers arrive at their interoperability design requirements. They claim that knowing “how” and knowing “which prerequisites” would enhance semantic interoperability between the different HIS is a solution. Obviously, contextual interoperability takes over as an important way of creating all round interoperability solutions.  The paper entitled “Targeting Perioperative Performance Aligned to Hospital Strategy via Digital Transformation” come from the US from our long- term contributors. Their focus is a digital transformation of an US hospital perioperative process, which yields targeted performance alignment to strategy. The research investigates existing limitations, potential capabilities and contextual understanding in order to address the complexity of perioperative processes. Consequently, research questions on how digital  transformation can assist in targeting perioperative performance have been answered: empowered individuals, integrated IS and digitally transformed processes allow for strategic and operational performance aligned to strategy.  The paper entitled “The Care2Report System: Automated Medical Reporting as an Integrated Solution to Reduce Administrative Burden in Healthcare” comes from the Netherlands, and focuses on the integrated solution for automating the process of medical reporting. The key characteristics of their software architectural dimensions are openness and modularity, which enable solutions suitable for technical and medical software applications’ perspectives. They use speech recognition technologies, linguistic annotations, semantic triples for knowledge graphs and ontologies, which in turn result in natural language generation components of the Care2Report system. The paper entitled “Process Innovation with Lightweight IT at an Emergency Unit”, illustrates the implementation of a lightweight IT support process innovation within an e-health information infrastructure, at a primary care emergency service in Oslo, Norway.  The authors demonstrate the role of lightweight technology in improving logistics and message interaction within and between health units. They suggest a “bypassing strategy” where a new layer of technology is built separately from the existing infrastructure in order to effectively address process innovation efforts. The paper entitled “A Workaround of EHR–A logistics Reporting System Development” illustrates the application of business process management for developing logistics and reporting system for laboratories, which service hospitals, medical centres, and physicians’ offices in the southern area of the United States. They also aim to improve web-based logistics and reporting system while maintaining HIPAA compliant controls.  In the paper entitled “Improving Perioperative Data Integrity and Quality via Electronic Medical Record Reconciliation” the authors illustrate the implementation of a study which investigates data integrity and quality within the perioperative process via embedded quality control check (QCC) rules.  Their business process management framework supports patient care documentation, performance reporting, patient billing, data analysis, and regulatory agency audits. This is a 166-month longitudinal study of a large 1,157 registered-bed academic medical centre, and therefore provides theoretical and practical implications and/or limitations of its results. The authors of the paper entitled “Exploring Multi-Modal Communication Approach for Young Children with Spinal Muscular Atrophy (SMA)” illustrate the development and implementation of a multi-modal communication approach for children affected by SMA.  They focus on a light-weight/ wireless microcontroller for processing electric signals from sensors and switches. The usability of various input devices are tested though an interactive game and a three-phase pilot study.  The authors of the paper “Assessing the impact of Physicians’ Virtual Communities on their medical Decision Making quality” assess the effect of physicians’ virtual community (VC) on the quality of their medical decision making. The participation of VC members is enabled through the application of the Social Capital Theory’s three dimensions, which assess the effectiveness of physicians’ VC.  The proposed model was empirically tested using an adapted survey for which data was collected from 204 SurveyMonkey VC physician members.  The last paper entitled “Software Architectures for Smart Applications in the Management of Chronic Diseases: A Study of Reversibility of Diabetes 2” illustrates the development of a software architectural model and its deployment in the domain of the personalised management of this chronic and metabolic disease. The implementation shows the synergy between patient medical records, their glucose readings and advice on the type of food which could be consumed in order to address the reversibility of the disease. They are very different and belong to a selection of problem domains in healthcare where software technologies either support or enable the efficiency of healthcare deliveries.       The paper Influencing Factors of Clinical Patient Recruitment Systems Design elaborates on the on the holistic design for clinical patient recruitment systems, for the purpose of defining critical success factors of clinical trial performances.  The authors defined these factors, and found new ones, through extensive literature review and several semi-structured expert interviews.  The outcome is interesting: their classification and systematization of influence factors for clinical patient recruitment systems, could be a foundation for specifying  requirements for modelling clinical patient recruitment systems in future.   The paper A multi-sensor system for enhancing situational awareness and stress management for people with ASD in the workplace and in everyday life, addresses a very important set of challenges autistic people face in their everyday lives. The paper illustrates the architecture and deployment of the project, which focuses on the positive behaviour support, within a customised environment, aiming to minimise negative impacts of deviations in factors like lighting or sound to the stress level of autistic people.  The outcome is a framework, which allows for constant monitoring of the environment and combining information from different sensor channels, including personal and environmental sources.  The architectural model is deployable as an application within mobile and wireless operating environments.  The paper Towards an Artificial Pancreas: Software Architectural Model and Implementation for Personalized Insulin Administration draws attention to the research field of developing an Artificial Pancreas and illustrates numerous problems related to data collection and their computations, which are supposed to mimic the functioning and performance of the natural organ.  The proposed software architectural model shows a new set of data and computations which address deficiencies of current insulation infusion pumps, used by millions of diabetic patient all over the world.  It also shows that an excerpt of the proposed architecture, deployed using Watson Analytics and performed upon a selection of data collected from sensors, individual patient’s input and persistent patient records, are far more powerful in understanding how to address insulin infusion, than any of the current solutions we have in the market and research laboratories. A shift in thinking and transdisciplinary research are needed to address the deficiency of insulin infusion pumps and computational models, which take into account only a very limited number of factors when calculating insulin infusion levels.       The paper A Case Study Perspective to the Digital Transformation of a Hospital’s Perioperative Process comes from our long-term contributors who explored implementations of software technologies across large hospital environments for the last 18 years.  In this submission, they look at a case study, which identifies existing perioperative sub-process limitations, potential capabilities, and subsequent sub-process contextual understanding to minimize perioperative process complexity.  They discovered that specific perioperative nursing documentation, plus electronic medical records, demonstrate the utility and value of patient-centric perioperative data.   Consequently the data plays the role of organizational resource for process management and control.  The paper also discusses, theoretical and practical implications of the case study.
