Development of a Checklist for Evaluating the Quality of Health Information on YouTube for Cancer Patients

학위논문(석사) -- 서울대학교대학원 : 간호대학 간호학과, 2023. 8. 김현의.유튜브의 사용자가 폭발적으로 증가하고 이에 따라 방대한 정보의 동영상이 업로드 되면서 유튜브는 의료 분야에서도 정보전달과 교육을 위한 중요한 플랫폼으로 부상하였다. 유튜브는 누구나 동영상을 만들어 업로드 하고, 손쉽게 시청할 수 있어 정보의 파급효과가 크다. 그러나 유튜브 동영상의 건강정보의 질은 보장되지 않아 취약한 암환자의 경우 잘못된 건강정보로 인해 생명에 위해를 입을 수 있어 유튜브 건강정보 질의 확인이 더욱 중요하다. 이와 관련하여 유튜브의 유용성과 유튜브 건강정보의 질 평가를 위한 다양한 연구가 이루어지고 있다. 그러나 표준화된 연구방법이 없고, 동영상 공유 소셜미디어인 유튜브의 특성을 반영한 유튜브 건강정보 질 평가 도구가 없었다. 또한 유튜브 건강정보 질의 개념에 대해 비슷한 의미의 여러 용어를 사용하며 정의도 명확하지 않았다. 따라서 본 연구는 암환자 자가간호를 위한 유튜브 건강정보 질의 개념을 명확히 하고 건강정보의 질을 평가하기 위한 체크리스트를 제안하고자 하였다. 본 연구에서는 3단계의 절차를 거쳐 암환자 자가간호를 위한 유튜브 건강정보 질 평가 체크리스트를 개발하였다. 첫 번째, 개념 분석을 실시하여 암환자 자가간호를 위한 유튜브 건강정보 질의 개념을 분석하고, 두 번째, 체크리스트 개발 과정에서는 개념 분석의 결과로 나타난 영역을 기반으로 초기문항 30개를 도출하였다. 이 초기문항에 대해 전문가 6인의 내용 타당도 검정과 가독성 평가를 거쳐 31개의 문항으로 수정, 보완하였다. 세 번째, 체크리스트 평가의 과정에서는 수정, 보완된 문항으로 15개의 유튜브 동영상 표본을 이용하여 전문가 2인의 평가를 통해 체크리스트의 신뢰도와 타당도를 검정하였다. 관찰자 간 일치도 평가와, 문항 총점간 상관관계, 두 그룹간 상관관계, 전문가의 주관적 평가와 평가 도구 점수와의 상관관계 검정을 통해 22문항의 최종 체크리스트를 개발하였다. 체크리스트는 중간 정도의 일치도(average kappa=.53)를 보였으며 신뢰도는 Cronbach's alpha 값이 .88로 안정성을 확보하였다. 본 연구는 암환자 자가간호를 위한 유튜브 건강정보 질 개념을 분석하고 확인된 평가영역과 지표로 체크리스트를 개발해 봄으로써 표준화된 도구를 개발하기 위한 노력을 시작했다는데 의의가 있다. 또한 본 연구를 통해 도출된 평가 영역과 지표를 고려하여 소비자의 요구를 만족시키는 질 높은 유튜브 동영상을 선별하여 제공하고 제작하는데 기준으로도 활용할 수 있을 것이다.YouTube has become a significant platform for information dissemination and education in healthcare, as its user base grows exponentially and a vast amount of videos containing medical information are uploaded. However, the quality of health information on YouTube is not guaranteed, posing a risk to vulnerable individuals, such as cancer patients. Consequently, evaluating the quality of health information on YouTube is of paramount importance. While numerous studies have explored the usefulness of YouTube and assessed the quality of health information, there is a lack of standardized research methods and specific tools tailored to YouTube's characteristics. Moreover, there is inconsistency in the terminology and definition of quality in YouTube health information. To address these gaps, this study aimed to clarify the concept of quality in YouTube health information for self-care among cancer patients and propose a checklist to evaluate the quality of health information in YouTube videos for cancer patients. The study employed a three-step approach to develop the evaluation tool. First, a concept analysis was conducted to analyze the concept of quality in YouTube health information for self-care among cancer patients. Second, based on the analysis, 30 initial items were derived during the checklist development process. These items were refined to 31 through expert validation and readability assessment. Finally, the revised checklist was evaluated for reliability and validity by two experts using 15 YouTube video samples. The assessment involved interrater reliability, item-total correlation, correlation between two groups, and correlation between experts' subjective evaluation and checklist scores. This process led to the creation of a final checklist consisting of 22 items, which exhibited moderate interrater agreement (average kappa = 0.53) and high reliability (Cronbach's alpha = 0.88), confirming its stability. This study clarifies the concept of quality in YouTube health information for self-care among cancer patients and provides a checklist for evaluating its quality.
