Experience of thrombolysis in the therapeutic time window in patients with acute ischemic stroke at Clínica Internacional, Lima is presented. An observational, descriptive, retrospective study was carried out for 5 years in 119 patients treated andhospitalized by the emergency service, of which 43 were acute ischemic cerebral events and only one met the criteria for emergency thrombolysis, having a favorable evolution and minimal sequelae. It is concluded that there is a need for a standardized protocol for the management of acute ischemic stroke, as well as the need for dissemination and education to the population for early recognition .Se presenta experiencia de trombólisis en la ventana de tiempo terapéutica en pacientes con accidente cerebral isquémico agudo en Clínica Internacional, sede Lima. Se realizó un estudio observacional, descriptivo, retrospectivo durante 5 años en 119 pacientes atendidos y hospitalizados por el Servicio de Emergencia, de los cuales 43 fueron eventos cerebrales isquémicos agudos y solo uno cumplió criterios para trombólisis de emergencia, teniendo una evolución favorable y mínimas secuelas. Se concluye en la necesidad de un protocolo estandarizado para el manejo del accidente cerebral isquémico agudo, así como también la necesidad de difusión y educación a la población para el reconocimiento precoz
