Biliary Enteropathy and its Surgical Treatment, Ileobiliary Unusual Case of Intestinal Obstruction, Case Report

Abstract

El íleo biliar es una complicación infrecuente, caracterizada por datos clínicos de oclusión intestinal, siendo el 4% de las causas de obstrucción intestinal en la población en general. El diagnóstico es clínico e imagenologico(tríada de rigler). El manejo al principio será conservador, sin embargo, al no tener adecuada evolución se requiere de intervención quirúrgica, siendo la enterotomía con extracción de cálculos la piedra angular. El objetivo de este reporte es presentar un caso de oclusión intestinal causado íleo biliar tratado mediante  laparotomía + enterotomía, extracción de lito y cierre primario. Se presenta el caso de una mujer de 69 años, mediante radiografía abdominal, colangioresonancia y tomografía abdominopelvica simple, se detectó enterocolito en íleon distal con datos de oclusión intestinal sin mejoría de tratamiento conservador. Se concluye que este diagnóstico es raro, pero de suma importancia en su diagnóstico para un abordaje terapéutico efectivo. The case of a 69-year-old woman is presented, by abdominal radiography, magnetic resonance cholangiography and simple abdominopelvic tomography, enterocolith was detected in the distal ileum with data of intestinal occlusion without improvement of conservative treatment.
