最低流量麻醉中七氟醚在两种挥发罐设定浓度下 的摄取规律及有效性观察

Abstract
【目的】 比较最低流量麻醉中七氟醚在体积百分数为8%与6%两种挥发罐设定浓度下的摄取规律及其临床实施的有效性65377;【方法】 气管插管全身麻醉下行胃肠道手术的患者40名,ASAⅠ-Ⅱ级,年龄20 ~ 60岁,按七氟醚挥发罐开启浓度(Fd)随机分为两组:8%组(G1组)和6%组(G2组),每组20例65377;经静脉诱导行气管插管接麻醉机后即将新鲜气流量调至0.5 L/min并维持在这一水平,按分组方法开启七氟醚挥发罐浓度至8%(G1组)或6%(G2组),维持呼气末CO2分压(EtCO2)于30 ~ 40 mmHg65377;每隔两分钟记录一般生命体征65380;七氟醚吸入浓度(Fi)65380;肺泡浓度(Fa,以呼末浓度表示)65380;最小肺泡浓度(MAC)值以及脑电双频指数(BIS),直至呼末七氟醚浓度达1.3 MAC (2.6%)65377;【结果】 开启挥发罐10 min后G1组的Fi65380;Fa值分别达(2.82 ± 0.21)%及(2.14 ± 0.20)%;G2组分别达(1.92 ± 0.17)%及(1.45 ± 0.18)%65377;两组Fi/Fd65380;Fa/Fd在265380;4 min时无差别;在665380;865380;10 min G1组高于G2组(P < 0.05);摄取期末G2组高于G1组(P < 0.05);G1组的Fa达1.3MAC的时间为(13.61 ± 2.12) min,G2组为(27.89 ± 7.62) min,差异有统计学意义(P < 0.001)65377;【结论】 七氟醚用于最低流量麻醉时,8%的挥发罐设定浓度可使呼吸道浓度在(13.61 ± 2.12) min即达到1.3 MAC,且BIS值已达到临床麻醉所需深度,心血管稳定性好,适于临床推广;而6%的挥发罐设定浓度则需(27.89 ± 7.62) min才能达到1.3 MAC,未能理想达到临床应用的要求65377
