intercellular adhesion molecule-1;nasal polyposis　　鼻息肉病(polyposis , nasal polyp disease)指那些具有鼻腔鼻窦黏膜广泛炎性水肿 、息肉样变 ,息肉呈多发复发倾向 ,激素治疗有效等特征的鼻息肉 ,其病因不明 。有报道细胞间黏附分子 1(inter-cellular adhesion molecule-1 , ICAM-1)与鼻息肉的发生发展有关[ 1] ,但与鼻息肉病的关系 ,鼻息肉组织 ICAM-1及鼻分泌物中可溶性细胞间黏附分子 1(soluble intercellular adhesion molecule-1 , sICAM-1)含量的关系均罕见文献报道。本文通过检测鼻息肉病鼻息肉组织中 ICAM-1 及鼻分泌物中sICAM-1的水平 ,以探讨 ICAM-1 及 sICAM-1 在鼻息肉病发病中的作用。1　材料和方法1.1　病例选择选择 1999年 10月～ 2001 年 1月在原中山医科大学附属第一医院耳鼻咽喉科住院病人 ,所有对象皆为非肿瘤患者。取标本的前 4周内无明显上呼吸道感染病史 ,未使用糖皮质激素和抗组胺药物;3 d内未服用抗生素。正常对照组组织取自健康志愿者中鼻道黏膜 ,共 5例 ,其中男性 3例 ,女性2例 ,年龄从 18 ～ 32 岁 ,平均年龄为 24岁 。鼻息肉组选择初发 、单发或多发但有明显蒂部者 ,并经病理确诊 ,共 8 例 ,其中男性 3例 ,女性 5 例 ,年龄从 17 ～ 37岁 ,平均年龄为 28岁。鼻息肉病的诊断依据参照顾之燕[ 2]文中有关内容 ,共 11 例 ,其中男性6例 ,女性 5 例 ,年龄从 13 ～ 73 岁 ,平均年龄为48岁;全部为双侧发病 ,前期手术史 2 ～ 4次不等 ,双侧鼻腔鼻窦黏膜有广泛性炎症反应和息肉样变性 ,鼻息肉 、息肉样变黏膜与正常黏膜无明显分界线 ,鼻窦 CT 扫描显示多发性鼻窦炎或全组鼻窦炎。1.2　取　材369中山医科大学学报(Acad J SUMS),2002 , 23(5):369～ 371DOI :10.13471/j.cnki .j .sun.yat -sen.univ(med.sci).2002.0126　　鼻分泌物采集方法见文献[ 3] 。鼻分泌物标本均在术前用镊子夹取棉片放入鼻腔 ,置于鼻道或息肉间取得 ,15 ～ 30 min 取出棉片 ,用注射器挤压出其中的鼻分泌物 ,用标有刻度的试管收集 1 ～ 2 mL液体(有 1例鼻息肉病患者鼻分泌物中混有血液而予以弃除)。立即置于 4 ℃冰箱并在 2 h内转到液氮中保存 。鼻息肉标本在术中取得 ,液氮中保存。1.3　方　法制备上清液:取出上述标本 ,室温解冻 ,将鼻分泌物置于混匀器上混匀 1 min后 ,4 ℃低温离心机 ,12 000 r/min , r =7 cm ,离心 5 min ,取上清液分装 ,置于 4 ℃冰箱备第 2天实验用。将鼻息肉剪成小块 ,称重;加入生理盐水(内含体积分数 0.1%的Tween-20液)配成 500 g/ L 的混悬液 ,冰浴下碾磨;全部倒入试管内 ,同上法 ,匀浆 、离心制备并保存上清液 。ELISA检测步骤按人 sICAM-1试剂盒(购自深圳晶美生物工程公司)说明书进行。1.4　计算与统计直线回归方程绘出 sICAM-1 标准曲线 ,得出各样本 ICAM-1和 sICAM-1 的含量(μg/ L),乘以稀释比 100∶1(组织匀浆上清)和 50∶1(分泌物上清),得出鼻分泌物原始浓度(μg/L),鼻息肉组织需进一步按 W∶V=1∶2的比例公式求得组织 ICAM-1的原始浓度(ng/g)。3组间 ICAM-1和 sICAM-1的含量差异的比较用方差分析和 SNK-q 检验;I-CAM-1和 sICAM-1含量的相关分析计算 pearson相关系数。以上统计工作皆应用 SPSS软件包完成。2　结　果鼻息肉病组鼻息肉组织匀浆 ICAM-1 及鼻分泌物中 sICAM-1的含量高于鼻息肉组和正常人对照组 ,见表 1 。鼻息肉病 、鼻息肉和正常人 3 组中 ,任何两组组织匀浆 ICAM-1 及分泌物 sICAM-1 含量的差异均有统计学意义 ,鼻息肉病组和鼻息肉组的组织匀浆 ICAM-1和鼻分泌物 sICAM-1呈强相关 ,相关系数达 0.74 ～ 0.87 ,并有统计学意义(P<0.05);正常对照组相关系数为 0.77 ,无统计学意义(P >0.05),见表 1。3　讨　论目前认为 ,鼻息肉的发生发展始终与嗜酸性粒细胞的聚集 、增多机制密不可分 cases in nasal polyposis group because 1 case w asmixed w ith blood in secretion and hadn' t been calculatedLFA-1)的黏附对嗜酸性粒细胞选择性黏附于血管壁 、穿过内皮细胞聚集于息肉组织过程中的第一步起关键性作用[ 1 , 4～ 6]。Tingsgaard等[ 5]将白细胞渗出过程细分为四步:①白细胞滚动接触 ,初次黏附于内皮细胞;②白细胞激活;③第 2次黏附(牢固黏附);④白细胞移出血管外。 ICAM-1 和 LFA-1 的配对在第 2次黏附中起重要的作用 。在机体正常状态下 , ICAM-1与配体 LFA-1呈低表达 ,当病变局部的细胞因子或炎症介质等释放时 ,组织中二者表达量增多 ,结合力增强 ,使大量嗜酸性粒细胞以及肥大细胞 、中性粒细胞等到达免疫反应及炎症部位 。本实验通过组织匀浆 ELISA 法检测发现鼻息肉组织中 ICAM-1含量明显高于正常鼻黏膜 ,结合临床上鼻息肉多合并鼻窦炎的事实 ,可能是由于鼻腔鼻窦的炎症刺激上调了 ICAM-1的表达 ,而 ICAM-1的高表达促使更多的炎性细胞浸润 ,这种恶性循环最终导致鼻黏膜向鼻息肉转变。有人用免疫组化的方法证明[ 6],鼻息肉组织中ICAM-1和 LFA-1表达较正常鼻黏膜显著增高 ,而且 ICAM-1的表达与 LFA-1 表达阳性的嗜酸性粒细胞浸润度呈显著正相关 ,表明鼻息肉组织的病理变化与 ICAM-1密切有关。Leonard等[ 7]发现从迁移的嗜酸性粒细胞中释放的产物 MBP(major basicprotein)、ECP(eosinophil cationic protein)可正反馈的提高人白细胞对鼻上皮细胞的黏附 ,也是通过白细胞 β2 整合素(CD11/CD18 包括 LFA-1 和 Mac-1)对鼻上皮 ICAM-1相互作用的增强实现的 。综上所述 ,嗜酸性粒细胞可能通过 ICAM-1引导 、吸附作用移行至血管外 ,然后再通过正反馈机制使其向组织内和黏膜下广泛浸润而得以充分发展 。鼻息肉病治疗棘手 ,病因复杂 ,定义也未统一。有学者将嗜中性粒细胞浸润为主的感染性息肉称370 中山医科大学学报(Acad J SUMS), 2002 , 23(5)为鼻息肉 ,嗜酸性粒细胞浸润为主的过敏性息肉称为鼻息肉病[ 8] 。后来又有人提出质疑 ,因为反映过敏的指标在两者之间并无显著性差别 。现在 ,更多的学者倾向于从临床特点上去定义和诊断鼻息肉病。本实验中鼻息肉病 ICAM-1的含量比正常对照和鼻息肉都高 , 有统计学意义 。由此推测 I-CAM-1在鼻息肉病的发生机制上有一定的作用 ,但促使鼻息肉病形成的机制有待进一步的研究。鼻分泌物主要为鼻腔浆液腺和杯状细胞分泌的产物 ,此外还有酶类 、细胞 、细胞因子等等。许多研究证明 ,鼻分泌物能很好的反映鼻腔疾病的状况[ 3 ,9] 。sICAM-1是由水解酶裂解血管内皮细胞 、上皮细胞及其它细胞膜上的 ICAM-1 使其胞外区脱落而成 ,因此 ,检测 sICAM-1水平可能成为监测某些疾病的动态指标。我们在鼻息肉手术前采集鼻分泌物 ,检测 sICAM-1的含量 ,发现鼻息肉组织ICAM-1和鼻分泌物 sICAM-1呈强相关 ,相关系数达0.74 ～ 0.87 ,并有统计学意义 ,说明鼻分泌物中sICAM-1的含量也能很好的反映鼻息肉组织中 I-CAM-1的表达情况 ,提示鼻分泌物中 sICAM-1 含量可用于了解鼻息肉病的疾病状况 ,作为鼻息肉病病情监测的指标之一 。参考文献:[ 1] Larsen P L ,
