Description of regional peribulbar anesthetic techniques for cataract surgery at the Fundación Oftalmológica de Santander - Clínica Carlos Ardila Lülle in the period from february to may 2015

Abstract

Durante los últimos años, se ha observado un cambio importante en cuanto a la cirugía oftálmica y las técnicas anestésicas utilizadas, hacia técnicas regionales y/o locales, permitiendo el aumento de las indicaciones quirúrgicas en los pacientes con mayores comorbilidades, obteniendo una mejor analgesia postoperatoria y comodidad, disminución en la incidencia de complicaciones postoperatorias, favoreciendo así la cirugía ambulatoria en este tipo de pacientes. La cirugía de catarata es el procedimiento oftalmológico más frecuente realizado por oftalmólogos a nivel mundial, siendo el tratamiento para la primera causa de ceguera reversible en el mundo. El rol del anestesiólogo ha venido en aumento en este campo, debido a que en años previos el oftalmólogo realizaba tanto la técnica anestésica como el procedimiento quirúrgico, con mayor riesgo de complicaciones. En las últimas décadas, las técnicas anestésicas oftálmicas han evolucionado hacia bloqueos peribulbares con el objetivo de mejorar la seguridad del paciente, asociado a la realización de estas en adecuadas condiciones permitiendo los mejores resultados posibles y disminuyendo al máximo posibles complicaciones. De este modo, el anestesiólogo ha participado en este ámbito para realizar una técnica adecuada, efectiva, cómoda para el paciente y cirujano, con adecuada monitorización y la mayor seguridad disponible. Es de vital importancia, el conocimiento minucioso de la anatomía y fisiología ocular, y de todas las técnicas anestésicas disponibles para ofrecer los mejores resultados. Como en la literatura no se encuentran estudios importantes con resultados precisos comparando la seguridad y/o efectividad entre las diferentes técnicas anestésicas a disposición, esta se escoge dependiendo de numerosas variables subjetivas, con importantes limitaciones principalmente debido a la falta de experiencia en la realización de las diferentes técnicas y a la ausencia de protocolos establecidos. Revisando los resultados promisorios en estudios con técnicas de punción única y el uso del bloqueo cantal medial como técnica alternativa única para la técnica de punción doble o la inyección única ínfero-temporal se hace importante la profundización en este tema.In recent years, an important change has been observed in terms of ophthalmic surgery and the anesthetic techniques used, towards regional and / or local techniques, allowing an increase in surgical indications in patients with greater comorbidities, obtaining better postoperative analgesia and comfort, reduction in the incidence of postoperative complications, thus favoring outpatient surgery in this type of patients. Cataract surgery is the most frequent ophthalmological procedure performed by ophthalmologists worldwide, being the treatment for the leading cause of reversible blindness in the world. The role of the anesthesiologist has been increasing in this field, due to the fact that in previous years the ophthalmologist performed both the anesthetic technique and the surgical procedure, with a higher risk of complications. In recent decades, ophthalmic anesthetic techniques have evolved towards peribulbar blocks with the aim of improving patient safety, associated with performing these in adequate conditions, allowing the best possible results and minimizing possible complications. In this way, the anesthesiologist has participated in this area to carry out an adequate, effective, comfortable technique for the patient and surgeon, with adequate monitoring and the highest available safety. Thorough knowledge of ocular anatomy and physiology, and of all available anesthetic techniques, is of vital importance to offer the best results. As in the literature there are no important studies with precise results comparing the safety and / or effectiveness between the different anesthetic techniques available, this is chosen depending on numerous subjective variables, with important limitations mainly due to the lack of experience in the performance of the different techniques and the absence of established protocols. Reviewing the promising results in studies with single puncture techniques and the use of medial canthal block as the only alternative technique for the double puncture technique or the single inferior-temporal injection, it is important to deepen this topic.1.
