Clinical efficacy and safety of acupuncture at Houtian Baguapoint around umbilicus combined with acupoint massage in the treatment of primary dysmenorrhea (针刺脐后天八卦穴联合穴位按摩治疗原发性痛经的临床疗效及安全性评价)

Abstract

Objective To observe the clinical efficacy and safety of acupuncture at the Houtian Bagua point around umbilicus combined with acupoint massage in the treatment of primary dysmenorrhea. Methods Totally75 patients with primary dysmenorrhea who met the inclusion criteria were randomly divided into control group (n=37) and treatment group (n=38). The control group was given Sanjie Analgesic capsule orally administration. The treatment group was given acupuncture at the Houtian Bagua point around umbilicus combined with acupoint massage. All patients were intervened for 3 menstrual cycles. The Cox Menstrual Symptom Scale(CMSS) and Visual Analogue Scale (VAS) were used to compare the dysmenorrhea status and pain degree of patients in the two groups, and the adverse reactions of patients in the two groups were compared. Results The total effective rate of the treatment group was71. 05%(27/38), which was significantly higher than 45. 95%(17/37)of the control group (P＜0. 05). After 3 menstrual cycles of treatment, the VAS score and CMSS score in the treatment group were significantly lower than those in the control group (P＜0. 05). There were 2 cases of constipation and 1 case of skin rash reported in the control group. There were 1 case of dizziness and 1 case of nausea reported in the treatment group. Conclusion Acupuncture at the Houtian Bagua point around umbilicus combined with acupoint massage can effectively improve the clinical symptoms of patients with primary dysmenorrhea, with high patient acceptance and few adverse reactions. (目的 观察针刺脐后天八卦穴联合穴位按摩治疗原发性痛经的临床疗效及安全性。方法 选取2021年1月—2022年10月医院就诊且符合纳入标准的原发性痛经患者75例, 按随机数字法分为对照组37例和治疗组38例。对照组给予散结镇痛胶囊口服; 治疗组给予针刺脐后天八卦穴, 同时配合特定穴位按摩。两组均治疗3个月经周期。利用痛经症状量表(CMSS)、视觉模拟评分(VAS)对比两组患者痛经状况及疼痛程度, 并对比两组患者不良反应。结果 治疗组总有效率71. 05%(27/38), 高于对照组的45. 95%(17/37), 差异有统计学意义(P＜0. 05)。治疗后3个月经周期后, 治疗组患者VAS评分和CMSS量表痛经持续时间、疼痛程度评分均较对照组降低明显, 差异有统计学意义(P＜0. 05)。对照组发生便秘2例, 皮肤瘙痒1例; 治疗组发生头晕1例, 恶心1例。结论 针刺脐后天八卦穴联合穴位按摩可有效改善原发性痛经患者临床症状, 患者接受度高, 不良反应少。
