Cameron, Edinburgh: I read the review byCruz-Herranz et al.,1 which emphasized the impor-tance of a clear protocol for optical coherence tomog-raphy (OCT) imaging and reporting in clinicalstudies relating to neurology. I agree with much ofthe reported checklist, but was surprised to see thatthe nomenclature for the outer retinal layers of theOCT image did not reflect recent agreements onstandardization of naming.An international consensus for nomenclature wasrecently published with comprehensive explanationsfor the naming choices, demonstrating a clear under-standing of the reflective mechanism of OCT tech-nology and how that relates to the cellular anatomyof the retina.2 In particular, the renaming of the layersrelated to the photoreceptor inner segments as zones(i.e., the myoid and ellipsoid zones).The authors may wish to update their recommendednomenclature to reflect the international consensus.Author Response: Philipp Albrecht, Düsseldorf,Germany; Andrés Cruz-Herranz, San Francisco;Axel Petzold, London; Wolf A. Brandt, Berlin: We thankDr. Cameron for the comment on our review andagree that additional structures can be differentiatedbelow the external limiting membrane (ELM) incomparison to what is illustrated in table 2 and thefigure.1 A myoid zone can be defined between theinner segment/outer segment junction (IS/OS) andthe ELM, an ellipsoid zone between the IS/OS andthe outer photoreceptor tips (OPT), and an interdig-itation zone between the OPT and the retinal pig-ment epithelium. These structures were detailed inthe International Nomenclature for OCT (IN-OCT) consensus article,2 and are relevant for researchconducted on the outer retina.The APOSTEL recommendations are a first steptowards consensus-based reporting guidelines forquantitative OCT studies, primarily focused on theinner retinal layers. However, it is essential to reachconsensus on nomenclature and reporting betweenresearchers from different disciplines and back-grounds, and with different retinal areas of interest.We are in the process of preparing a formalizedconsensus-building approach, which involves a largerpublic, including ophthalmologists, neurologists, andbasic scientists to further refine, evaluate, and revisethe recommendations. We intend to include themore detailed nomenclature of the structures belowthe ELM and reference the IN-OCT consensus arti-cle in future versions of the recommendations.© 2016 American Academy of Neurology1.
