1. Behandeling van functionele bewegingsstoornissen en de BoNT studieBotuline NeuroToxine (BoNT) heeft zich bewezen als effectieve behandelmethode in verschillende bewegingsstoornissen (1-4). Dit proefschrift beschrijft een gerandomiseerde placebo-gecontroleerde trial (RCT) naar het effect van BoNT vs. placebo in FBS. In de RCT-fase werd aangetoond dat 64% van de patiënten verbetering van motorische symptomen liet zien na behandeling met BoNT, vergeleken met 57% in de placebogroep. Dit verschil was niet statistisch significant. In de niet-geblindeerde fase van de studie nam de verbetering van motorische symptomen toe naar 81%. Psychiatrische co-morbiditeit bleek frequent aanwezig, waarbij angststoornissen en functionele pijnstoornissen het vaakst voorkwamen. Er werd bij patiënten met motorische verbetering na behandeling een verbetering van angststoornissen geobserveerd. Op de lange termijn liet de meerderheid van de patiënten een aanhoudende motorische verbetering zien.2. Functionele bewegingsstoornissen en de schrikreflexGestoorde stress regulatie en een verhoogde staat van alertheid spelen een belangrijke rol in de pathofysiologie van FBS (5,6). Om dit pathofysiologisch fenomeen te onderzoeken, werd de auditieve schrik reactie (ASR) onderzocht in FBS. De uitkomsten lieten een verhoogd aantal zowel vroege als late responsen zien bij patiënten met FBS. Het patroon van de responsen was echter niet afwijkend. Zowel een verhoogde staat van alertheid als een gedragsmatige component werden geacht een rol te spelen in de gevonden resultaten. Na behandeling met BoNT bleek dat de vroege respons, geassocieerd met abnormale stress regulatie, normaliseerde in combinatie met het verbeteren van motorische symptomen. De late respons echter bleef verhoogd in patiënten vergeleken met controles ondanks behandeling, passend bij het persisteren van de afwijkende gedragsmatige component
