We consider a system subject to hard and soft failures in which a hard failure is preceded by a hard defect and an in-service failure occurs either when a hard failure occurs or the number of soft failures reaches some critical level. A periodic preventive maintenance policy is cost-optimized for this system. The model we develop is motivated by maintenance planning for railway rolling stock and for train doors in particular. We present some results from this context
