GIANT OVARIAN MUCINOUS CYSTADENOMA

Rezumat
Obiective. Tumorile mucinoase ovariene sunt o variantă a neoplasmelor epiteliale și constituie de la 10% până la 15% în structura neoplasmelor
ovariene primare.
Prezentare de caz. Tumorile mucinoase ovariene gigante sunt un fenomen destul de rar și sunt prezentate în literatura de specialitate ca observații
sporadice. Având în vedere raritatea tumorilor mucinoase ovariene gigante, vă prezentăm propria noastră observație clinică.
Pacienta C., 24 ani, a fost spitalizată pentru tratament chirurgical cu formațiune de volum gigantă depistată timp de 5 luni. Acuză dureri în regiunea
inferioară a abdomenului, mărirea în volum al abdomenului. La internare starea relativ satisfăcătoare. Abdomenul mărit în dimensiuni din contul
unei formațiuni de volum din bazinul mic care ajunge până la procesul xifoid. Pentru aprecierea volumului intervenției chirurgicale, stratificării
formațiunii a fost efectuată tomografie computerizată, la care s-a depistat la nivelul Th12 - S5, posibil cu origine din ovarul stâng, formațiune chistică
masivă de o configurație nedeterminată, bine conturată, septată, polilobulară, cu conținut lichidian omogen, densitatea nativă a conținutului +10
UH, dimensiunile 24.8x18.3x12.6 cm fără semne de infiltrație a țesuturilor adiacente, posibil chistadenom mucinos. S-a luat decizia de a efectua
intervenție chirurgicală. La revizia cavității abdominale s-a depistat o formațiune de volum, multicamerală 20x20 cm. S-a efectuat anexectomie.
Pacienta a fost externată la a 9-a zi postoperator.
Concluzii. Tumorile mucinoase ovariene gigante sunt o patologie destul de rară. În tratamentul chirurgical al Tumorile mucinoase ovariene gigante,
ar trebui să se acorde preferință intervențiilor chirurgicale ovaromenajante la pacientele de vârstă reproductivă.Summary
Objectives. Mucinous ovarian tumors are a variant of epithelial neoplasms and constitute from 10% to 15% of the structure of primary ovarian
neoplasms.
Case presentation. Giant mucinous ovarian tumors are a rather rare phenomenon and are presented in the specialized literature as sporadic
observations. Given the rarity of giant mucinous ovarian tumors, we present our own clinical observation.
Patient C., 24 years old, was hospitalized for surgical treatment with detected giant volume formation for 5 months. It causes pain in the lower
abdomen, the increase in volume of the abdomen. At admission the condition is relatively satisfactory. The abdomen increased in size due to a
volume formation in the small pelvis that reaches the xiphoid process. In order to assess the volume of the surgical intervention, the stratification of
the formation, a computed tomography was performed, where a massive cystic formation of an indeterminate, well-contoured, septate, polylobular
configuration was detected at the Th12 - S5 level, possibly originating from the left ovary with homogeneous liquid content, native content density
+10 HU, dimensions 24.8x18.3x12.6 cm without signs of infiltration of adjacent tissues, possible mucinous cystadenoma. The decision was made
to perform surgery. During the revision of the abdominal cavity, a 20x20 cm multicameral volume formation was detected. An adnexectomy was
performed. The patient was discharged on the 9th postoperative day.
Conclusions. Giant mucinous ovarian tumors are a rather rare pathology. In the surgical treatment of giant mucinous ovarian tumors, preference
should be given to ovarian-preserving surgery in women of reproductive age
