For policymakers ‘to build back better’ systems to face future shocks,they will need to be able to determine which areas work best(in terms of providing access, quality, population health,responsiveness or efficiency) to prioritize and direct resourcestowards. More than ever, we need now to count with stronghealth systems monitoring, appraisal and assessment to drawpractical policy implications.This plenary will look at the effective application of HealthSystems Performance Assessment (HSPA) to health systems’improvement as we face key challenges in the sustainability ofour health systems. Too often the results of HSPA exercises,particularly when benchmarking is involved, are not wellinterpreted, and understood. When translating HSPA resultsinto policy changes, we need to address a series of questionsnot only about the quality and validity of the indicators but,importantly, about the causal attribution and accountabilityimplications and about the kinds of policy interventionsrequired to address the performance failures.Following an introductory keynote providing practical illustra-tions of those issues and drawing policy lessons, the panelists willfocus on three key HSPA questions particularly relevant in thecurrent policy context. First, how to interpret and attributeoverall health system performance outcomes to individual healthsystem functions and strategies in need of reform. Second, howbest to benchmark and compare performance between Europeancountries to identify and learn from best practices. These diseases usually manifestthemselves later in life, but they are not necessarily related tobiological ageing. They mostly result from an accumulation ofunhealthy living patterns since childhood and across the lifecourse. Consumption of processed foods high in fat, salt, andsugar, smoking, excessive alcohol use, too little physicalactivity and too much stress all contribute to the growingand worsening burden of chronic diseases. These behaviours inturn are shaped by the social, environmental, cultural andeconomic conditions in which we live, grow, work and age.Once they have developed, chronic diseases can be difficult, oreven impossible to cure. Our health services, with a traditionalcurative approach, are not equipped for this chronic epidemic.There is an urgent need to shift our health services away fromthe predominant focus on cure and towards prevention, andfor policy makers to invest in ensuring healthy livingenvironments and societies. Health promotion and enablingpeople and population groups to increase control over theirhealth, particularly those facing disadvantage, has the potentialto transform our health services, and is critical to ensuringtheir resilience and sustainability.Despite a growing awareness of the need for change,reorienting structures and systems in practice is challenging,as people can be resistant to change. Siloed approaches withinthe health sector, but also between social, health, andeducation sectors continue to prevail, and it is not alwayseasy to find the right levers for change and to build bridgesacross administrations. This is compounded by a lack ofinfrastructure, organizational and workforce capacity forhealth promotion, and sustainable financing mechanisms.Much innovative work is however taking place, which we canlearn from and scale up.This plenary session will provide examples of different ways inwhich health-promoting approaches can reorient healthservices, strengthen health-promoting and communityoriented primary care and prevent chronic diseases. It willhighlight what we can learn from behavioural and culturalinsights and social prescribing, as well as integrated commu-nity initiatives to further support people, across the socialgradient, to lead and to maintain healthy lives.
