Revista da Sociedade Brasileira de Medicina Tropical22(4): 167-170, Out-Dez, 1989.EDITORIALCITOCINAS, INTEGRINAS EPATOGENIA DA MALÁRIAAo iniciar o ciclo eritrocitário, o plasmódio desencadeia um complexo conjunto de reações infla­matórias e imunológicas decorrentes da ativação de macrófagos33 35^  ]infócitos T 1^  B' e células NK2^. Em conseqüência, várias citocinas são produzidas e liberadas por estas células, inclusive as interleucinas (IL), interferons (IFN) e fatores de necrose tumoral (FNT), além de diversos fatores de crescimento. As citocinas, agindo através de suas ações autócrinas e parácrmas, em concentrações mínimas, são essenciais para a indução e a regulação da resposta imune. Dados experimentais e clínicos sugerem que a interação do parasito com o hospedeiro pode apresentar uma evolução binomial. Em um pólo, as citocinas podem apresentar atividade antiplasmódio, como ocorre com o fator de necrose tumoral-alfa (FNT-alfa)14 32. Esta citocina apresenta em comum com a IL-1 e IL-6 a capacidade de induzir febre, síntese de proteínas da fase aguda, aumento de atividade procoagulante23 ativação de fagócitos e de células endoteliais^ 23 Contudo, no outro pólo, estas mesmas citocinas, quando produzidas em excesso, podem estar envolvi­das na fisiopatogenia da malária grave. Grau e cols11 mostraram em camundongos inoculados com P. berghei, que apenas nos animais com níveis séricos elevados de FNT-alfa ocorria desenvolvimento de malária cerebral. O uso de anticorpos monoclonais (AcM) anti-FNT-alfa11 ou anti-IFN-gama12 impede o desenvolvimento das lesões cerebrais. Além disso, a anemia24, a diseritropoiese8 e o aborto associado à malária? podem ser induzidos e/ou mediados, pelo menos em parte, pelo FNT-alfa.Existe similaridade entre o quadro clínico da sepse e o da malária grave^. No entanto, até o momento, ainda não foi definitivamente estabelecida a semelhança na fisiopatogenia das duas condições clínicas. A exemplo da sepse, em que ocorre envolvi­mento de toxinas, também na malária ocorre intensa produção e liberação de FNT-alfa2 3 20 27 31.Apesar do grande desenvolvimento dos conhe­cimentos sobre a fisiopatogenia da malária, muitas questões ainda permanecem sem resposta. Por que em uma parcela de pacientes com malária aguda apre­senta aumento nos níveis séricos de FNT-alfa, como demonstrado em vários trabalhos8 13 20 30? A resposta a esta pergunta necessitará de uma definição mais clara da cinética e da dinâmica das interações entre as citocinas. O achado recente de inibidores naturais de citocinas18 22 e da presença de anticorpos naturais anti-FNT ou IL-14 em indivíduos normais,CYTOKINES, INTEGRINS AND THE PATHOGENESIS OF MALARIAMalaria parasites bring about, during the erythrocyte cycle, a complex body of inflammatory and immune reactions, due to the activation of macro­phages33 35, T 19 and B1 lymphocytes and NK cells2 .^ As a consequence, several cytokines are produced and released by these cells, such as interleu­kins (IL), interferons (IFN) and tumour necrosis factors (TNF), together with some growth factors. These cytokines act, in minute concentrations, through autocrine and paracrine actions, in the induction and modulation of immune response. Some experimental and clinical data suggest that the interactions between malaria parasite and the host cells may follow a binomial feature. In one hand cytokines may display antiplasmodial activity as happens with TNF- alpha14 32. Similar to TNF-alpha, II^ l and IL -6 are capable to induce fever, the synthesis of acute phase proteins, show procoagu lant activity23, and activate both phagocytes and endothelial cells8 23. On the other hand, when the production of these cytokines is enhanced they may play a role in the pathogenesis of complicated malaria. Grau et al have demonstrated that mice inoculated with P. berghei and showing nervous system involvement presented high levels of serum TNF-alpha. Monoclonal antibodies against TNF alpha11 or gamma-IFN12 were able to protect infected animals from the development of cerebral malaria Moreover, some of the pathological features of malaria such as anemia24, dyserythropoiesis7 and abortion^ may be caused, at least in part, by TNF- alpha.There are some similarities between the clinical picture of severe malaria and sepsis^, although it has not been possible to establish whether the same factors are involved in the pathogenesis. As in sepsis, high levels of serum TNF-alpha are detected in ma- laria2320 27 31.Some important questions remain unanswered, for instance: why only some patients with acute malaria present increased levels of serum TNF-alpha, as demonstrated by several authors7 132030? A better knowledge of the kinetics, and of the dynamics of the interactions between different cytokines is needed. The recent finding of natural serum inhibitors of cytokines18 22, and of the occurrence of an ti-IH  and anti-TNF-alpha antibodies in normal people4 may help us to understand individual susceptibility to severe and complicated malaria.The sequestration of parasitized erythrocytes within small vessels is a hallmark of the pathogenesis167Editorial. Revista da Sociedade Brasileira deMedicina Tropical 22: 167-170, Out-Dez, 1989.que aumentam após determinados estímulos infeccio­sos, pode concorrer para um melhor entendimento sobre a susceptibilidade individual para o desenvolvi­mento de quadros graves de malária.O seqüestro de eritrócitos parasitados na micro- circulação constitue uma das características funda­mentais da patogenia da malária pelo P.falciparum. A ligação destes eritrócitos com as células endoteliais tem sido atribuída, em grande parte, à propriedade de algumas moléculas de superfície destas células. As mais conhecidas são as CD-36, a trombospondina e a CI>54 (ICAM-1)17. Esta última faz parte do grupo das integrinas, receptores para moléculas de aderência celular16, processo essencial para a citotoxicidade mediada por células, a fagocitose, a quimiotaxia, a indução da proliferação e diferenciação celular, a domiciliação celular e aderência às células endo- teliaislfi 24 Na malária, as integrinas são fundamen­tais para o desenvolvimento de lesões, inclusive as cerebrais. O uso de AcM antimoléculas de aderência impede a ligação das hemácias parasitadas às células endoteliais17. Por outro lado, citocinas como FNT- alfa, ILl-alfa, ILl-beta e IFN-gama podeni modular a densidade de receptores e ligantes de aderência na superfície celular de fagócitos ou de células endote- liais2^. Recentemente, em estudo prospectivo de 65 crianças malauianas, com malária por P. falcipamm, a concentração sérica inicial do FNT-alfa foi maior nos pacientes qpe faleceram13. Houve também, uma associação entre as concentrações desta citocina e hipoglicemia, hiperparasitemia e o desenvolvimento de malária cerebral. Todavia, ainda resta a demons­trar, o envolvimento in vivo de citocinas e integrinas na lesão das células endoteliais nos casos de malária grave.As perspectivas de utilização dos conhecimen­tos das citocinas e integrinas na patogenia da malária são enormes: 1) o desenvolvimento de uma vacina contra exoantígenos que estimulem as citocinas cons­titui uma possibilidade muito atraente-^, principal­mente quando se considera que o desenvolvimento de vacinas antiparasitárias tem logrado somente êxito par­cial até o momento; 2) a manipulação nutricional é um outro aspecto importante, já  que ácidos graxos essenciais10 ou micronutrientes (zinco e cobre)29 podem modular a produção e liberação de ILl-alfa e beta e de FNT-alfa; 3) o uso de fármacos capazes de interferir com a produção ou ação das citocinas. Apesar de os corticosteróides serem capazes de inibir a produção de FNT-alfa21, dois estudos controlados demonstraram ser desfavorável o seu uso nos casos de malária cerebral15 36 Isto provavelmente decorre, em parte, do fato de que estes fármacos atuem somente na etapa pré-transcripcional do FNT-alfa21. Existe, en­tretanto, drogas capazes de atuar em níveis pós- transcripcionais, como a pentoxifilina3^, ou até mes­mo ao nível dos receptores das citocinas9.of P. falciparum  malaria. The binding of these ery­throcytes to endothelial cells involves the presence of adhesion molecules and their respective receptors, including CD-36, thrombospondin, and CD-54 (ICAM-1)17. This latter protein belongs to the family of the integrins and behave as receptors for adhesion molecules responsible for phenomena as diverse as cell-mediated cytotoxicity, phagocytosis, chemotaxis, cell proliferation and differentiation, homing of lymphocytes, and binding to endothelial cells30. Inte­grins probably play a crucial role in pathogenesis of malaria, since monoclonal antibodies to adhesion molecules block the binding of parasitised erythrocytes to endothelial cells17. Furthermore, some cytokines as TNF-alpha, IL-1 alpha and gamma-IFN modulate the density of receptors and ligands of adherence on the surface of phagocytes and endothelial cells28.This new information on the pathogenesis of malaria may open up new avenues for the management of this infection: 1) Development of vaccines using some exo-antigens capable to stimulate cytokine pro­duction26; 2 )  A nutritional modulation of cytokine synthesis through essential fat acids10 or micro­nutrients such as zinc and copper29; 3) The use of drugs, which can interfere with either the production or the action of cytokines. Corticosteroids are capable of inhibiting the synthesis of TNF-alpha, but at a pre- transcriptional level21. Indeed, two controlled trials showed no benefit in the usage of corticosteroids for the treatment of cerebral malaria15 36. Some drugs, however, such as p e n to x y p h i l in 34  may block TNF- alpha at a post-transcriptional level34, or even at the level of its receptors10.It is possible to anticipate that a better know­ledge of the role played by cytokines and integrins in the pathogenesis of malaria will contribute to improve the prevention and treatment of severe and compli­cated cases of this infection.REFERÊNCIAS BIBLIOGRÁFICAS1.
