However,the equipments for CCP were unreliable, costly anddifficult to transport between hospitals. Therefore,during the past 20 years, clinical organ transplantationacross the UK and Europe has developed on the basisof  the ‘cold flush’ method where organ  have beenpreserved on ice after vascular flush with  specially-designed cold flush solutions. The main cold flushsolution has been University of Wisconsin solution, butothers have also been developed. However, over theyears, the success of organ transplantation has meantthat there are growing numbers of patients who couldbenefit from transplantation, but a shortage of suitabledonor organs. This has led to a great  pressure toexpand the criteria for accepting less-than-optimalorgans which have experienced considerable warmhypoxia before cold preservation (the so-called non-heart beating donor  – NHBD organs). Experimentalevidence has shown that continuous cold perfusion withoxygenated solutions can repair some of the damagewhich took place in the NHBD organs  and so thepreservation period can be used to ‘rescue’ andimprove these organs to achieve good results intransplantation. This has led to the development in thepast 2 years of a new generation of hypothermicperfusion machines for kidneys and livers, which arebeing studied in trials in Europe. These developmentswill be discussed.
