Het doel van dit proefschrift was om bij te dragen aan de kennis van effectief beleid om geneesmiddelengebruik te verbeteren, door het analyseren van de impact van de recente hervorming van de gezondheidszorg in China. In dit proefschrift beschrijven we eerst de status en trends van het Chinese systeem van gezondheidszorg, alsmede de huidige situatie en de problemen van het Chinese farmaceutische systeem. Wij presenteren ook onze beweegredenen voor het maken van een analyse van de invloed van de verschillende componenten van het hervormingsbeleid op het gebruik van medicijnen. De volgende onderzoeksvragen komen aan de orde:1. Wat zijn de sterke en de zwakke kanten van het Chinese farmaceutische systeem?2. Wat zijn de effecten van klinische educatieve interventies op het gebruik van geneesmiddelen, en vooral op het gebruik van antibiotica?3. Wat zijn de effecten van verschillende soorten financiële hervormingen op het gebruik van gezondheidszorg, en van geneesmiddelen?4. Wat zijn de effecten van geïntegreerde hervormingen op het gebruik van gezondheidszorg, en van geneesmiddelen?De resultaten van dit proefschrift laten zien dat universele toegankelijkheid tot basisgezondheidszorg en essentiële geneesmiddelen grotendeels is gerealiseerd in China. Het is nu vooral belangrijk dat de juiste prikkels worden geschapen, door middel van zorgvuldig ontworpen financieringsmechanismes van de gezondheidszorg in zowel de aanbodzijde als de vraagzijde. Hiermee kan de efficiëntie en de kosteneffectiviteit van de gezondheidszorg worden verbeterd. Effectieve gebundelde betalings systemen met verschillende componenten zijn nu nodig, alsmede een uitgebreider gebruik van indicatoren en doelstellingen om de kwaliteit van de zorg te bewaken binnen het gekozen “pay-for-performance” systeem.The aim of this thesis was to contribute to the knowledge of effective policies to improve use of medicines by analyzing the impact of the recent health system reform policies in China. In this thesis, we first describe the status and trends of the Chinese health system, as well as the current situation and problems of China's pharmaceutical system. We also present our rationale for making impact analysis of various components of the reform on the use of medicines. The following research questions were addressed:1. What are the general strengths and challenges of the Chinese pharmaceutical system?2. What are the effects of clinical educational interventions on medicines use, and especially on the use of antibiotics?3. What are the effects of various financing reforms on the use of health services and medicines within the health system reform framework?4. What are the effects of integrated system reforms on the use of health services and medicines within the health system reform framework?The results of this thesis show that universal access to basic health-services and essential medicines has largely been achieved in China. It is now particularly important that the right incentives are created through carefully designed financing mechanisms in both supply and demand side. It is now one of the top tasks for China to improve the efficiency and cost-effectiveness of health care. Designing an effective bundled payment system with mixed approaches, as well as a comprehensive pay-for-performance indicator system with specific quality of care targets is needed
