Abstract
Background. Recent neuroimaging acquisition techniques
have demonstrated significant assessment benefits and
appropriate triage of patients based on specific clinical and
radiological features in the acute stroke management. Objective
of the study. The objective of this study is to share
the latest imaging tools in evaluation and diagnosis of an
acute ischemic stroke. Their impact on management and
clinical course of the stroke is also described. Material and
Methods. This work explores the use of multimodality imaging
for treatment selection of patients with AIS in the context
of recent guidelines, highlighting controversial topics
and providing guidance for clinical practice. The advantages
and disadvantages of CT, CTA, MRI, and MRA in stroke diagnosis
are summarized with attention to the role of vascular
imaging and perfusion imaging. Results. Various imaging
modalities are routinely used for diagnosis and management
of acute ischemic stroke, including multimodal computed
tomography (CT) and magnetic resonance imaging
(MRI). These imaging methods should provide information
beyond the presence or absence of intracranial hemorrhage
as well as the presence and extent of the ischemic core,
collateral circulation, and penumbra in patients with neurological
symptoms. Differences between the extension of the
ischemic core and the extension of penumbra may optimize
selection of patients with late or unknown symptom onset
who would potentially be eligible for endovascular treatment
therapy. Conclusion. We present a comprehensive
review of the current evidence about efficacy and theoretical
basis of present imaging modalities, and explores future
directions for imaging in the management of acute ischemic
stroke.Introducere. Metodele neuroimagistice moderne au demonstrat
un beneficiu enorm în triajul pacienților cu ictus
ischemic acut, oferind în baza datelor radiologice și clinice
puncte cheie în managementul acestor pacienți. Scopul
lucrării. Analiza ultimelor investigații radiologice utilizate
în diagnosticul și evaluarea unui pacient cu ictus ischemic
acut. Impactul acestor investigații în tratamentul și evoluția
clinică a pacienților este la fel descris. Materiale și Metode.
În lucrare sunt descrise metodele imagistice de bază utilizate
în ictusul ischemic acut: CT, CTA, RMN, angio-RMN cu
accent pe imagistica vasculară cerebrală și studiile de perfuzie.
Este redată și utilitatea clinică a acestor investigații prin
selecția pacienților pentru tratamentul de revascularizare
specific în baza recomandărilor protocoalelor și ghidurilor
existente. Rezultate. Investigațiile radiologice contemporane
vin să ofere informații complexe despre un pacient cu
ictus ischemic acut. Astfel, suplimentar informației despre
prezența sau absența unei hemoragii intracraniene se obțin
date despre focarul ischemic format, circulația colaterală
și extinderea zonei de penumbră. Diferența dintre zona de
ischemie formată și zona de penumbră este crucială în selecția
pacienților cu timp de debut necunoscut pentru tratamentul
endovascular de revascularizare cerebrală. Concluzii.
