Pe lângă pre-paratele specifice citopatogenice a fiecărei patologii inflamatorii orale, pe larg sunt folosite, și anume pri-mele, remediile simptomatice de combatere a durerii — semnului timpuriu a inflamaţiei.Compania farmaceutică Bosnalijek (Bosnia-Her-ţegovina) a elaborat și propune spre utilizare un pre-parat antiinfecţios bucal cu numele Lysobact DUO compus din clorhidrat de lisozimum 20mg și clorură de ceptilpiridinium 1,5 mg în formă de comprimate. Componenţa comprimatelor Lisobact DUO este ști-inţific argumentată prin rezultatele efective și origi-nale obţinute pe parcursul anilor în tratamentul ma-ladiilor infecţioase-inflamatorii a mucoaselor căilor respiratorii superioare, în terapia complexă a astmu-lui bronșic și pneumoniei, asanarea focarelor cronice de infecţii stafilococice.Lisozimul este un ferment prezent în salivă și alte lichide biologice a organismului uman. Un rol antibacterian însemnat manifestă lisozimul în lichi-dul lacrimal, prezentând o concentraţie până la 7000 mkg/ml, relativ mai scăzută în laptele mamelar (până la 300 mkg/l). Besides spe-cific cytopathogenic medicine that are used for each oral inflammatory pathology, first are widely used symptomatic remedies for pain control — the early sign of inflammation.The pharmaceutical company Bosnalijek (Bos-nia–Herzegovina) has developed and proposes for use an oral drug called Lysobact DUO, composed of clorhidrium lysozyme 20mg and cetylpyridinium chloride 1,5mg in tablet form. The composition of Lysobact DUO tablets is scientifically based for its effective and original results obtained over the years in the treatment of infectious–inflammatory diseases of the upper respiratory tract mucosa, in the com-plex therapy of bronchial asthma and pneumonia, the healing of chronic outbreaks of staphylococcal infections.Lysozyme is an enzyme found in saliva and other biological fluids of the human body. A significant an-tibacterial role manifests lysozyme in the tear fluid, having a concentration up to 7000 mkg/ml, relatively lower in breast milk (up to 300 mkg/l). The content of lysozyme in the egg albumin (4500 mkg/l) is high, and it manifests antimicrobial protection in the de-veloping embryo.According to Navasin S. (1982), the distribution of 51Conform datelor lui Navasin S. (1982), repartizarea lisozimului în elemente celulare sanguine neutrofilare și monocitare, capabile să manifeste efectul fagocitozei, prezintă rolul lui antibacterian semnificativ. Autorul determină devieri apreciabile de la indicii activităţii li-sozomale sanguine în cadrul unor leziuni infecţioase și somatice, inclusiv și în cele în regiunea maxilo-facială.Cunoscut este faptul că, cavitatea orală se deo-sebește prin gradul major de infecţie microbiană a mucoaselor gingivale, jugale, în conţinutul pungilor gingivo-odontale. La persoanele sănătoase, supra-tensionarea bacteriană semnificativă se echilibrează prin mecanismele de protecţie eficace, manifestată prin cicatrizare „per secundam intentionem“ și regle-mentare permanentă a cantităţii societăţilor microbi-ene în firida biologică dată.Este confirmat că, procesele de protecţie locală, în majoritatea cazurilor sunt condiţionate de lisozim, care se conţine în concentraţii semnificative în salivă. Multiple procese patologice, manifestate în cavitatea orală, evoluează pe fundalul scăderii activităţii enzi-mului dat, fapt care insistă de suplimentat cantitatea lui prin aplicări medicamentoase exterioare.Această enzimă hidrolizează conexiunea dintre acidul N-acetilmuramic și N-acetil-D-glucozamină, rezultând în distrugerea peretelui bacterian. Compo-nenta a doua a Lysobact DUO, cetilpiridinium acţio-nează ca protectoarea mucoasei cavităţii orale.Indicaţiile producătorului de folosire a Lysobact DUO sunt stomatitele, gingivitele, afecţiunile herpe-tice și eroziv-aftoase ale cavităţii orale.Scopul investigaţiei clinice a constituit studierea eficacităţii Lysobact DUO, atât separat cât și în aso-ciere cu alte preparate de acţiune patogenică, pentru etioterapia unor forme de stomatite infecţioase orale.Au fost trasate următoarele obiective:1. Estimarea folosirii Lysobact DUO în tratamen-tul antibacterian a gingivitei catarale acute și a unor stomatite infecţios-virale;2. Aprecierea eficacităţii antidolore a Lysobact DUO în unele forme de stomatită infecţios-virală.Material și metode de cercetareAu fost supuși tratamentului antiinfecţios 47 de pacienţi, cu vârsta cuprinsă între 15 și 45 de ani, de ambele genuri (35 feminin și 12 masculin). Pacien-ţii au prezentat stomatită traumatică — 13 persoane; stomatită herpetică — 22; 13 pacienţi — gingivită ca-tarală acută.Fiecare pacient utiliza câte 1-2 comprimate la fie-care 6-8 ore (recomandarea producătorului) timp de 4-8 zile. Comprimatele erau ţinute sub limbă, până la dizolvarea lor completă. Pe parcursul tratamentului simptomatic nu au fost determinate efecte de hiper-sensibilitate la utilizarea Lysobact DUO. Putem cu fermitate menţiona că, Lysobact DUO potenţa acţi-unile antiviralelor locale și a altor remedii medica-mentoase etiopatogenice aplicate.lysozyme in neutrophilic and monocyte blood cell el-ements, capable of manifesting the effect of phagocy-tosis, has its significant antibacterial role. The author determines significant deviations from the indices of blood lysosomal activity in infectious and somatic le-sions, including those in the maxillofacial region.It is known that the oral cavity is distinguished by the major degree of microbial infection of the gingival, buccal mucous membranes, in the contents of the gingivo–odontal pockets. In healthy people, significant bacterial burden is balanced by effective protective mechanisms, manifested by “per secondam intentem” healing and permanent regulation of the amount of microbial quantities within the given bio-logical lineage.It is confirmed that local protection processes in most cases are conditioned by lysozyme, which is contained in significant concentrations in saliva. Multiple pathological processes, manifested in the oral cavity, evolve on the background of decreased activity of the enzyme, which insists on supplement-ing its quantity by external drug applications.This enzyme hydrolyzes the bonds between N–acetylmuramic acid and N–acetyl–D–glucosamine resulting in the destruction of the bacterial wall. The second component of Lysobact DUO, cetylpyri-dinium acts as a protector of the mucosa of the oral cavity.The manufacturer’s indications for using Lyso-bact DUO are stomatitis, gingivitis, herpes and ero-sive–aphthous disorders of the oral cavity.The purpose of the clinical investigation was to study the efficacy of Lysobact DUO, both separately and in combination with other pathogenic prepara-tions, for the etiotherapy of some forms of oral infec-tious stomatitis.The following objectives have been identified:1. Estimation of the use of Lysobact DUO in the antibacterial treatment of acute catarrhal gingivitis and infectious–viral stomatitis;2. Assessment of the pain–relieving efficacy of Lysobact DUO in some forms of infectious–viral sto-matitis.Research material and methodsUnderwent anti–infectious treatment 47 patients aged between 15 and 45 years, of both genders (35 fe-male and 12 male). Patients with traumatic stomatitis — 13 persons; herpetic stomatitis — 21; 13 patients with acute catarrhal gingivitis.Each patient uses 1–2 tablets every 6–8 hours (manufacturer’s recommendation) for 4–8 days. The tablets were kept in the oral cavity dissolved under the tongue. There were no hypersensitivity effects to the use of Lysobact DUO during symptomatic treat-ment. We can firmly mention that, Lysobact DUO potentiated the actions of local antivirals and other etiopathogenic medicinal remedies applied.52Rezultate și discuţiiLysobact DUO este un remediu medicamentos antibacterian, micșorând gradul edemaţiei mucoa-sei, manifestă și o ușoară acţiune analgezică. Pe lângă dezinfectarea mucoasei cavităţii orale și a faringelui rezultă și efect antidolor. Mai mult, nu se exclude acţiunea Lysobact DUO de distrugere a fungilor și a dezvoltării virusurilor. Patogenetic Lysobact DUO ameliorează evoluţia inflamaţiei locale, prevenind dezvoltarea unor infecţii bacteriene mai agresive. Re-mediul dat, fiind relativ ușor solubil în lichidul oral, poate penetra suficient și sectoarele mucoasei orale slab accesibile, inhibând multiplicarea bacteriană.Concluzii:1) Lysobact DUO este un remediu contemporan antibacterian „cruţător“, care poate fi aplicat pe larg în stomatologia terapeutică la tratarea simtomatică a „-itelor“ mucoasei orale.2) Lysobact DUO posedă de acţiuni antidolore, potenţionând scăderea edemului inflamator în urma efectului etiopatogenic a cetilpiridi-niumului.3) Lysobact DUO prezintă eficacitate de pene-trare antibacteriană în zonele mucoasei orale puţin accesibile, manifestată prin micșorarea gradului de inflamaţie și dolorităţii în zonele respective.Results and discussionsLysobact DUO being an antibacterial medicine remedy also shows a slight analgesic action, reducing the degree of mucosal edema. In addition to disin-fecting the mucosa of the oral cavity and the pharynx, there is also an analgetic effect. Moreover, the action of Lysobact DUO by destroying the fungi and arrest-ing the development of viruses is not excluded. Patho-genetically, Lysobact DUO improves the evolution of local inflammation, preventing the development of more aggressive bacterial infections. The remedy, be-ing relatively easily soluble in the oral fluid, can pen-etrate sufficiently the poorly accessible oral mucosal sectors as well, inhibiting bacterial multiplication.Conclusions1) Lysobact DUO is a contemporary “gentle” antibacterial remedy, which can be widely applied in therapeutic dentistry to the symp-tomatic treatment of oral mucosal stomatitis.2)  Lysobact DUO possesses pain–relieving ac-tions, potentially reducing inflammatory edema following the etiopathogenic effect of cetylpyridinium.3) Lysobact DUO has antibacterial penetration efficacy in areas of poorly accessible oral mu-cosa, manifested by decreasing inflammation and pain in the respective area.Bibliografie/References:1.
