The choice of treatment modality in metastatic breast cancer depends on an age, performance status of the patient, comorbidities, disease dynamics, localization and number of metastatic lesions as well as an expression of hormonal receptors and HER2 receptor. Here we present the case of hormone receptor-positive postmenopausal metastatic breast cancer patient with the massive extension of the disease at presentation who were treated with fulvestrant, an estrogen receptor antagonist, after the failure of initial chemotherapy. Treatment with fulvestrant was associated with a significant improvement in physical status of the patient, weight gain, mobility and reduction of bone pain. Moreover, remission of the metastatic disease was durable.Wybór sposobu leczenia przerzutowego raka piersi zależy od wielu czynników, takich jak: stan ogólnej sprawności pacjenta, choroby współistniejące, wiek, lokalizacja i liczba ognisk przerzutowych. Decyzja terapeutyczna zależy przede wszystkim od statusu receptorów hormonalnych i receptora HER2. Prezentowany przypadek dotyczy pomenopauzalnej chorej z masywnymi przerzutami hormonozależnego raka piersi leczonego fulwestrantem, po uprzednim niepowodzeniu chemioterapii. Leczenie fulwestrantem wiązało się ze znaczącą poprawą stanu ogólnego, zwiększeniem masy ciała, poprawą aktywności pacjentki oraz wyraźnym zmniejszeniem nasilenia bólu.
