Toiminnallisen opinnäytetyön aiheena oli kotihoidon kehittäminen tutkimalla asioita, joita kotihoidossa työskentelevän sosiaali- ja terveydenhuollon ammattilaisen on hyvä tietää mennessään kotikäynnille romanivanhuksen kotiin. Opinnäytetyössä aineistonkeruun yhteydessä tutkittiin myös asioita, joita romanikulttuurin omaava asiakas toivoisi hoitajan huomioivan tullessaan kotikäynnille. Toiminnallisessa opinnäytetyössä pohdittiin myös seikkoja, joilla varmistettaisiin laadukas ja asiakaslähtöinen palvelu ja palvelutarpeen arviointi romanikulttuurin omaavalle vanhusasiakkaalle. Tutkimuksen osaongelmia oli 1) vähäinen romanikulttuurin tuntemus kotihoidossa, 2) kulttuurin tuomien erityispiirteiden huomioiminen, 3) tasa-arvoisen ja syrjimättömän kohtelun takaaminen ja 4) asiakaslähtöisyyden toteutuminen romaniasiakkailla. Opinnäytetyö tehtiin syksyn 2015 ja kevään 2016 välisenä aikana.
Opinnäytetyön teoreettisina lähtökohtina käsiteltiin romanien historiaa, romanikulttuuria ja siitä nousevia erityispiirteitä. Tässä osuudessa käsiteltiin myös kotihoitoa palveluna, asiakaslähtöisyyden periaatteita ja toteutumista sekä kulttuurista kompetenssia hoitotyössä. Opinnäytetyön lähtökohtien osuus pohjautuu vahvasti kirjallisuuteen romanikulttuurista, sekä ammattikirjallisuuteen.
Toiminnallisen opinnäytetyön tuotoksena tehtiin opaskirja ”Romanivanhus kotihoidon asiakkaana - opas kotihoidon henkilökunnalle”. Oppaaseen oli tarkoitus kerätä tietoja siitä, millaisia piirteitä romanikulttuurissa on, joita kotihoidon työntekijän on hyvä tietää hoitaessaan romanivanhus asiakasta. Oppaaseen oli myös koottu tietoa asioista, joita haastattelujen perusteelle nousi esiin niistä asioista, joita romanikulttuurissa elävät toivovat itse kotihoidon työntekijän tietävän. Oppaasta haluttiin tehdä mahdollisimman käytännönläheinen sekä asiakasta että työntekijää palveleva.
Oppaaseen kerättiin tietoa kirjallisuuden ja haastattelujen perusteella asioista, kuten romanivanhuksen palvelutarpeen arvioinnista, sekä päivittäisten toimintojen että sairaanhoidon toteuttamisesta. Oppaaseen tuli myös tietoa kotihoidossa mahdollisesti esiintyvistä erityistilanteista, kuten muistisairaan kohtaamisesta, mielenterveys- ja päihdeongelmista, syrjinnästä, kuolevan asiakkaan kohtaamisesta, sekä uskonnosta ja hengellisyydestä. Lopuksi oppaaseen haluttiin ottaa esille asioita, jotka antavat työkaluja kulttuuriosaamiseen kotihoidon henkilökunnalle. Opas antaa tietoa myös asiakaskeskeisestä työskentelystä, kuntouttavasta työotteesta, osallisuudesta ja haavoittuvuudesta, sekä kulttuurisesta kompetenssista.The purpose of the thesis was to study topics which a social and health care profes-sional should know when making a home call to an elderly Roma person and what the Roma themselves hoped to be taken into consideration when a nurse makes a home call. The aim was to assess the need for services and to find out how good quality client-centred services could be provided to elderly Roma people. The prob-lems related to the study involved the following things 1) scarce knowledge of the Roma culture from the perspective of home care 2) attention to specific cultural fea-tures 3) ensurance of equal and non-discriminatory treatment and 4) implementation of client-centredness among Roma clients. The thesis was carried out by using multi-ple approaches during autumn 2015 and spring 2016.
The theoretical part of the thesis deals with the history of Roma people, the Roma culture and the specific issues related to the culture. The theoretical part also de-scribes home care as a service, the principles of client-centredness and their imple-mentation in nursing the Roma as well as cultural competence in nursing. The theoret-ical part relies strongly on literature on the Roma culture and professional literature.
The thesis work resulted in a guide called Elderly Roma as home care clients – a guide for home care personnel. The guide was compiled on the basis of literature and interviews. The guide describes those features in Roma culture which a home care worker should know when nursing elderly Roma clients. In addition, the guide in-cludes facts derived from the interviews with the Roma clients on the issues they think the home care workers should know. The aim was to make a practical guide which would serve both the clients and the professionals.
The guide includes information of the need for services among Roma clients and how daily activities and nursing should be carried out. In addition, it provides information of specific situations in home care, for example meeting a Roma person with a memory disease, mental health problems or substance abuse problems. The guide also discusses issues related to religion, spirituality and dying patients. Tools are provided to improve the personnel’s cultural competence, to take a client-centred and rehabili-tation approach to work as well as participation and vulnerability
